精神分裂症患者血清HS-CRP、NLR水平与精神症状及认知功能的相关性

目的 探讨精神分裂症患者血清超敏C反应蛋白(High-sensitivity c-reactive protein HS-CRP)、中性粒细胞/淋巴细胞比值(Neutrophil-to-lymphocyte ratio NLR)与认知功能及精神症状的相关性.方法 选取上海市宝山区精神卫生中心2021年12月~2022年8月新住院精神分裂症患者42例为研究组,同时选取42例基本资料相匹配的健康体检组为对照组;两组受试者入组后均使用重复性成套神经心理状态测验(The Repeatable Battery for the Assessment of Neuropsychological status,RBANS)、阳性和阴性症状量表(Positive and Negative Symptom Scales PANSS)评定认知功能和精神症状,抽取静脉血后检测两组血清HS-CRP水平、中性粒细胞及淋巴细胞水平,并计算NLR值.结果 相较于正常对照组,研究组的HS-CRP水平及NLR值均升高(HS-CRP:0.37±0.05ug/dl vs 0.46±0.36ug/dl;NLR:1.67±0.48 vs 2.79±1.01),差异有统计学意义(P<0.01);与对照组相比,精神分裂症患者认知功能总分及各因子分均降低,差异有统计学意义(P<0.05);分析研究组,患者的HS-CPR水平表达与认知功能中即刻记忆因子分(r=-0.362,P<0.05)呈负相关,与症状量表中一般病理量表因子分(r=0.369,P<0.05)呈正相关;NLR值与即刻记忆因子分(r=-0.394,P<0.05)呈负相关,与阴性症状(r=0.39,P<0.05)呈正相关;而分析研究组PANSS量表及RBANS量表只显示患者的阳性、阴性症状与认知功能中注意因子呈负相关(r=-0.521,-0.37,P<0.05),余均无相关性.结论 精神分裂症患者血清中HS-CRP的表达水平及NLP值均高于正常对照组,且与精神症状严重程度及认知功能下降有关,主要体现在即刻记忆水平及注意障碍等方面,且精神分裂症患者的精神症状越重,注意功能越差.
Correlation of serum HS-CRP and NLR levels with psychiatric symptoms and cognitive function in pa-tients with schizophrenia
Objective To investigate the correlation of serum High-sensitivity c-reactive protein(HS-CRP)and Neutrophil-to-lymphocyte ratio(NLR)with cognitive function and psychiatric symptoms in patients with schizo-phrenia.Methods A total of 42 newly hospitalized schizophrenic patients from Shanghai Baoshan Mental Health Cen-ter from December 2021 to August 2022 were selected as the case group,while 42 healthy patients with matched basic data were selected as the control group.Participants in both groups were assessed for cognitive function and psychiatric symptoms using The Repeatable Battery for the Assessment of Neuropsychological statuss(RBANS)、Positive and Neg-ative Symptom Scales(PANSS),and venous blood samples were taken.Serum HS-CRP、neutrophils and lymphocytes levels in both groups were detected after blood drawing,then calculate the NLR value.Results Compared with nor-mal control group,HS-CRP level and NLR value of case group were increased(HS-CRP:0.37±0.05ug/dl vs 0.46±0.36ug/dl;NLR:1.67±0.48 vs 2.79±1.01,the difference was statistically significant(P<0.01).Compared with the control group,the total score of cognitive function and each factor score of schizophrenia patients were decreased,and the difference was statistically significant(P<0.05).Analysis of the case group,the expression of HS-CPR level was negatively correlated with the score of immediate memory factor in cognitive function(r=-0.362,P<0.05).There was a positive correlation with the general pathological scale factor score(r=0.369,P<0.05)in the symptom scale.NLR values were negatively correlated with immediate memory factor scores(r=-0.394,P<0.05)and positively correlated with negative symptoms(r=0.39,P<0.05).The PANSS scale and RBANS scale showed a negative correlation between positive and negative symptoms and attention factor in cognitive function(r=-0.521,-0.37,P<0.05),the rest were not correlated.Conclusion The expression level of HS-CRP and NLP in serum of schizophrenic patients were higher than those of normal control group,its were related to the severity of mental symptoms and the decline of cognitive function,mainly reflected in the level of immediate memory and attention disorders,and the more severe symptoms of schizophrenic patients,the worse attention function.
