目的 探讨首发青少年抑郁症患者应用认知疗法联合经颅磁刺激治疗的临床效果.方法 于2020年3月~2023年3月采集病历资料入档,对象为医院收治的青少年抑郁症患者共计纳入290例,纳入对象基于"随机数字表法"1:1分组,划分为对照组(予以常规药物治疗)与观察组(予以常规药物+认知疗法联合经颅磁刺激),各纳入145例;观察对比两组心理评分、睡眠质量、认知功能、血清脑源性神经营养因子水平.结果 治疗后,两组汉密尔顿抑郁量表(Hamilton Depression Rating Scale,HAMD)评分均呈下降趋势,且观察组较对照组更低(P<0.05);治疗后,两组青少年睡眠习惯问卷(the Adolescent Sleep Habits Questionnaire,ASHQ)评分均有不同程度降幅,且观察组降幅较对照组更大(P<0.05);治疗后,两组蒙特利尔认知功能评估量表(the Montreal Cognitive Function Assessment Scale,MoCA)评分均升高,观察组评分高于对照组(P<0.05);治疗后,两组血清脑源性神经营养因子(Brain-derived neurotrophic factor,BDNF)水平均呈升高趋势,且观察增加值较对照组明显高(P<0.05).结论 认知疗法联合经颅磁刺激用于治疗首发青少年抑郁症患者,可发挥调控机体神经营养因子的作用,使患者抑郁程度得到显著改善,进一步提高其睡眠质量,且对患者认知功能提高有显著效果.
The effect of cognitive therapy combined with transcranial magnetic stimulation in first-episode adoles-cent depression patients
Objective To explore the clinical efficacy of combination therapy with transcranial magnetic stimu-lation in first-episode adolescent depression patients.Methods Case data were collected and filed from March 2020 to March 2023.A total of 290 adolescent depression patients admitted to the hospital were included,and the partici-pants were divided into two groups based on the"random number table"method.They were divided into the control group(treated with conventional medicine)and the observation group(treated with conventional medicine+cognitive therapy combined with transcranial magnetic stimulation),145 cases were included respectively;the psychological score,sleep quality,cognitive function,and serum brain derived neurotrophin level of the two groups were observed and compared.Results After treatment,the scores of Hamilton Depression Rating Scale(HAMD)in both groups showed a downward trend,and the observation group was lower than the control group(P<0.05).After treatment,the ASHQ scores of both groups of adolescents decreased to varying degrees,and the observed group showed a greater decrease than the control group(P<0.05).After treatment,the scores of the Montreal Cognitive Function Assessment Scale(MoCA)in both groups increased,and the scores of the observation group were higher than those of the con-trol group(P<0.05).After treatment,the level of serum brain-derived neurotrophin(BDNF)in both groups showed an upward trend,and the observed increase value was significantly higher than that in the control group(P<0.05).Conclusion Cognitive therapy combined with transcranial magnetic stimulation can play a role in regulating the neu-rotrophin in the treatment of first-episode adolescent depression,which can significantly improve the depressive degree of patients,further improve their sleep quality,and have a significant effect on the improvement of cognitive function of patients.
