目的 探讨老年抑郁症患者应用微生态制剂辅助认知行为干预(Cognitive behavioral therapy,CBT)的临床价值。方法 选取2021年2月~2023年2月在我院门诊收治的160例老年抑郁症患者,随机分为观察组(微生态制剂辅助CBT,80例)和对照组(CBT,80例)。记录两组汉密尔顿抑郁-17(Hamilton depression-17,HAMD-17)量表评分和简易精神状态评价量表(Simple Mental State Assessment Scale,MMSE)评分,检测两组肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、高敏 C 反应蛋白(high-sensitive C-reactive protein,hs-CRP)以及白细胞介素-6(Interleukin-6,IL-6)水平,观察两组不良反应。结果 治疗后两组HAMD-17评分均下降,观察组低于对照组(P<0。05);治疗后两组患者TNF-α、hs-CRP、IL-6水平显著下降,观察组低于对照组(P<0。05);治疗后两组MMSE评分均上升,观察组高于对照组(P<0。05);两组不良反应总发生率比较无显著差异(连续性校正x2=2。025,P=0。155)。结论 微生态制剂辅助CBT能有效减轻老年抑郁症患者的抑郁程度,降低炎症反应,改善认知功能,安全性高。
Effect of cognitive behavioral intervention with microecological agents in elderly patients with depression
Objective To investigate the clinical value of Cognitive behavioral therapy(CBT)assisted by mi-croecological agents in elderly patients with depression.Methods A total of 160 elderly patients with depression ad-mitted to our outpatient hospital from February 2021 to February 2023 were selected and studied,and were randomly divided into observation group(80 cases of microecological-assisted CBT)and control group(80 cases of CBT).Ham-ilton depression-17(HAMD-17)score and Simple Mental State Assessment Scale(MMSE)score were recorded in the two groups.Tumor necrosis factor-α(TNF-α),high-sensitive C-reactive protein(hs-CRP)and interleukin-6(IL-6)levels were detected in the two groups,and adverse reactions were observed in the two groups.Results HAMD-17 scores of both groups decreased after treatment,and the observation group was lower than the control group(P<0.05).After treatment,the levels of TNF-α,hs-CRP and IL-6 in two groups were significantly decreased,and the obser-vation group was lower than the control group(P<0.05).MMSE scores in both groups increased after treatment,and the observation group was higher than the control group(P<0.05).There was no significant difference in the total inci-dence of adverse reactions between the two groups(continuity correction x 2=2.025,P=0.155).Conclusion Micro-ecological preparations assisted CBT can effectively reduce the degree of depression in elderly patients with depression,reduce inflammatory response,improve cognitive function,and have high safety.
