The impact of disease uncertainty and optimistic tendencies on self health management in patients with myasthenia gravis

目的 分析疾病不确定感、乐观倾向对重症肌无力(Myasthenia Gravis,MG)患者自我健康管理的影响.方法 2020年12月～2023年7月,选取MG患者97例,采用中文版疾病不确定感量表(Mishel Uncertainty in Illness Scale,MUIS)、生活定向测验修订版(Revised Life Orientation Test,LOT-R)、成年人健康自我管理能力测量表(Adult Health Self-Management Scale,AHSMSRS)评估疾病不确定感、乐观倾向、自我健康管理,分析MG患者MUIS、LOT-R评分与AHSMSRS评分的相关性,以及乐观倾向在疾病不确定感与自我健康管理中介效应.结果 MG患者MUIS、LOT-R、AHSMSRS评分分别为(101.18±13.76)分、(16.03±4.13)分、(115.74±15.24)分.经 Pearson 分析,MG患者MUIS各维度评分及总分与AHSMSRS各维度评分及总分呈负相关(P＜0.05),LOT-R评分与AHSMSRS各维度评分及总分呈正相关(P＜0.05).使用Bootstrap抽样检验法进行中介效应研究,AHSMSRS总分=＞LOTR评分=＞MUIS总分,此条中介路径存在效应.结论 MG患者乐观倾向较低,在疾病不确定感和自我健康管理之间有中介作用,需提高患者的乐观倾向,从而降低疾病不确定感,提高自我健康管理.
Objective Analyze the impact of disease uncertainty and optimistic tendencies on self health man-agement in myasthenia gravis(MG)patients.Methods From August 2020 to July 2023,97 MG patients were se-lected and evaluated for disease uncertainty,optimistic tendencies,and self health management using the Chinese version of the Mishel Uncertainty in Illness Scale(MUIS),Revised Life Orientation Test(LOT-R),and Adult Health Self Management Scale(AHSMSRS)The correlation between LOT-R score and AHSMSRS score,as well as the medi-ating effect of optimistic tendencies on disease uncertainty and self health management.Result The MUIS,LOT-R,and AHSMSRS scores of MG patients were(101.18±13.76),(16.03±4.13),and(115.74±15.24),respectively.According to Pearson analysis,the MUIS scores and total scores of MG patients were negatively correlated with the AHSMSRS scores and total scores(P＜0.05),while the LOT-R score was positively correlated with the AHSMSRS scores and total scores(P＜0.05).Using the Bootstrap sampling test method for mediating effects research,AHSMSRS total score=＞LOTR score=＞MUIS total score,this mediating path has an effect.Conclusion MG patients have a low tendency for optimism,which mediates between disease uncertainty and self health management.Therefore,it is nec-essary to improve the patient's optimism,thereby reducing disease uncertainty and improving self health management.
