艾司氯胺酮在小儿麻醉中的应用进展

摘要
艾司氯胺酮是集镇静、镇痛、麻醉于一体的N-甲基-D-天冬氨酸受体(NMDAR)拮抗剂,具有起效迅速、代谢快、副作用小等特点,临床常用于程序化镇静、围手术期、减轻癌痛和抑郁症患者等.由于儿童药理学代谢特点及临床用药经验不同于成年人,文章针对艾司氯胺酮应用于小儿麻醉,对其药理学作用及临床应用经验进行综述,着重阐述了该药物在儿童体内的代谢、对呼吸循环和神经系统的影响、不同的临床适用场景和药物相关不良反应的发生情况,旨在为临床儿童患者治疗的药物选择提供理论依据和参考.
Abstract
S-ketamine is an N-methyl-D-aspartate receptor(NMDAR)antagonist that combines sedation,analgesia,and anes-thesia.It has the characteristics of rapid onset,rapid metabolism and few side effects,and is often used in clinical programmed sedation,perioperative period,to alleviate cancer pain and depression patients.As children's pharmacological and metabolic characteristics and clinical drug experience are different from those of adults,this article reviews the pharmacological effects and clinical application expe-rience of esketamine in pediatric anesthesia,focusing on the metabolism of this drug in children,its effects on respiratory circulation and nervous system,different clinical application scenarios and the occurrence of drug-related adverse reactions.The aim is to provide theoretical basis and reference for drug selection in the treatment of clinical children.
