正颌手术中气管导管缝线致气管套囊漏气1例

摘要
正颌手术术中需要上下颌咬合对位,一般采用经鼻气管内插管,为防止气管导管的脱落,常用丝线固定气管导管于鼻小柱上.文章报道1例正颌手术中气管导管缝线致气管套囊漏气,该患者术中出现气管导管漏气,给予口咽部纱条填塞,输注泵持续给套囊内泵注空气,患者顺利完成手术,术毕患者清醒拔管,无误吸,肺部听诊双肺呼吸音清.术后5 d安全出院.
Abstract
Orthognathic surgery requires the upper and lower jaw occlusal alignment,where anesthesia intubation is commonly adopted through nasotracheal intubation.In order to prevent the endotracheal tube from falling off,a silk thread is often used to fix the endotracheal tube on the nasal columella.The articles reports one case of endotracheal tube cuff leakage caused by endotracheal tube suture during orthognathic surgery.The patient had air leakage from the endotracheal tube during the operation.Accordingly,the oro-pharynx was packed with gauze,and the infusion pump continuously pumped the air into the cuff pump.The patient completed the op-eration successfully.At the end of the operation,the patient was awaked and extubated,without aspiration,and the respiratory sounds were clear in both the lungs on lung auscultation.The patient was discharged five days after the operation.
