Analysis on the Effect of Using Gray Correlation Methods to Improve Clinical Pathway Management under the Background of DRG Reform
Objective To explore the key management tasks in the implementation of hospital clinical pathways to improve the effectiveness of clinical pathway implementation and provide guarantee for the hospital's Diagnosis Rela-ted Groups(DRG)payment reform.Methods The gray correlation analysis method was used to analyze the cases in-cluded in clinical pathway management in a pulmonary hospital in Wuhan from January to December 2019.Key indi-cators were selected to strengthen management.Clinical pathway-related indicators such as coverage,access rates,average length of stay,and average hospitalization costs before and after management were compared.The rank sum test was used to compare the differences between disease type(group)weight/score(relative weight,RW)interval groups of patients who completed clinical pathway management in 2019,2020 and 2021.Results The factors affec-ting clinical pathway implementation efficiency were ranked from large to small as coverage rate(0.953 4)>entry rate(0.948 2)>average length of stay(0.942 5)>variant completion rate(0.805 8)>normal completion rate(0.782 8)>mutation exit rate(0.718 9).After strengthening the management of key tasks,the coverage rate of clini-cal pathway-related indicators,entry rate,and entry completion rate had all increased,and the average length of stay had decreased.The proportion of patients with RW interval grouping ≥2 who completed clinical pathway manage-ment in 2019,2020 and 2021 had increased year by year,and the average hospitalization cost had remained sta-ble.Conclusion Gray correlation analysis helps to clarify the key tasks in the development of clinical pathways,strengthens the management of key indicators,has a good supervisory effect,and further promotes the effective im-plementation of DRG reform.
