Current Situation and Factors Affecting the Demand for Long-term Care of Homebound Elderly in Lingshui County,Hainan Province
Objective To understand the current situation of long-term care needs of the elderly at home in Ling-shui County,Hainan Province by analyzing the factors influencing the long-term care needs of the elderly,thus to propose countermeasures and suggestions for improvement.Methods Firstly,a stratified random sampling was used to conduct a questionnaire survey on 911 homebound elderly in six townships of Lingshui County,Hainan Province between August and October 2021.Then,to understand the current situation of long-term care needs of homebound elderly in Lingshui County,the current situation of long-term care needs of homebound elderly was graded by using the Probit value combined with the commonly used graded number method.Finally,single-factor analysis was con-ducted by Kruskal-Wallis H-test,and an ordered logistic regression analysis was used for multi-factor analy-sis.Results A total of 930 questionnaires were distributed and 911 valid questionnaires were collected in this study,with a valid return rate of approximately 98%.The evaluation results showed that there was little difference between the proportion of mild depen-dence and severe dependence after adjustment,and the proportion of moderate depend-ence was the highest.The Kruskal-Wallis H test suggested that age,household registration,main source of income,health insurance,degree of self-care,the number of having chronic diseases,hypertension,diabetes,cerebrovascular diseases,medication status,family history and falls(≤ 3 months)had statistically effects on long-term care needs of older adults(P<0.05).The ordered logistic regression suggested that age(relative to the 80 years age group,60-<65 years:OR=7.352;65-<80 years:OR=3.658),main economic sources(children of economic sources:OR=0.666),medical insurance category(the new rural cooperative medical scheme:OR=1.944),degree of self-care(self-care:OR=7.000)were factors associated with long-term care needs of the elderly(P<0.05).Conclusion The need for health care was the highest among homebound elderly in Lingshui County;primary financial source,type of health insurance,degree of self-care,exercise,frequency of breakfast consumption,dietary taste-sour,and dietary tastesweet were the factors affecting the long-term care needs of homebound elderly.
