Analysis on the Problems and Optimization of the Automatic Peritoneal Dialysis Utilization:Based on the Anderson Health Service Utilization Model in China
Objective To systematically analyze the application of automatic peritoneal dialysis(APD)in China and to pro-pose optimization ideas.Methods Under the guidance of Adelson health service utilization behavior model,NVIVO software was used to analyze the collected literature and interview data,sort out the existing problems in the application of APD techno-logy,and deduce the development and optimization ideas backward.Results From the four dimensions of environmental fac-tors,personal characteristics,dialysis behavior and dialysis results,25 APD technology application problems were summarized,including insufficient cognition and poor compliance of patients with APD,medical staff often recommended hemodialysis to pa-tients with inertial thinking,and the low level of medical insurance payment compared with hemodialysis.Based on these ele-ments,mechanisms and systems,the authors put forward three optimization ideas:considering the separate effects of each di-mension,considering the mechanism of action between different dimensions,and considering the closed-loop design of the sys-tem.Conclusion The low utilization rate of APD technology is a complex system problem.We should not only consider the possible interaction between different stakeholders and different dimensions in the application of APD technology,but also find the right entry point and solve the prominent problems in the application of APD technology step by step based on the realistic situation.
