Analysis of Factors Influencing Insomnia in Hospitalized Patients with Coronary Heart Disease and the Effectiveness of the Psycho-cardiology Intervention Model
Objective To explore the influence of psycho-cardiology intervention model on sleep status and psycho-logical status among in-patients with coronary heart disease(CHD)complicated with insomnia,and to analyze the in-fluencing factors of insomnia in inpatients with CHD.Methods 635 inpatients with CHD admitted to department of cardiology of the First Affiliated Hospital of Air Force Medical University from May 2021 to December 2023 were selected.According to the evaluation results of Pittsburgh Sleep Quality Index(PSQI)scale,the patients were divided into insomnia group(PSQI score>7 points,n=406)and good sleep group(PSQI score ≤7 points,n=229).Patients in the insomnia group were divided into two groups by simple randomization method.The control group was given routine department intervention model,while the observation group received psycho-cardiology intervention mo-del.Univariate analysis and multivariate Logistic regression analyses were used to analyze the influencing factors of insomnia in in-patients with CHD.Differences in Pittsburgh Sleep Quality Index(PSQI)scores,coping styles,Hamil-ton Anxiety Scale(HAMA)scores,Stanford Acute Stress Response Questionnaire(SASRQ)scores,and Self-rating Anxiety Scale(SAS)scores were compared between intervention modes.Results Univariate analysis and multivariate Logistic regression analysis showed that ward environment discomfort(OR:3.861;95%CI:1.685-8.847),severe psychological burden(OR:1.960;95%CI:1.685-8.847),severe disease discomfort symptoms(OR:4.289;95%CI:1.821-10.100),indoor noise or bright light(OR:1.614;95%CI:1.047-2.490),and severe postoperative recovery burden(OR:3.501;95%CI:1.646-7.445)were the influencing factors of insomnia in inpatients with CHD(P<0.05).The scores of dissociation,provocation,avoidance,re-experiencing,and functional impairment after 1 and 6 months of treatment were lower in the two groups compared to before treatment,and lower in the observation group compared to the control group(P<0.05).PSQI score,HAMA score,and SAS score were reduced in both groups af-ter 1 month and 6 months of treatment than before treatment,and lower in the observation group than in the control group(P<0.05).Conclusion The insomnia of inpatients with CHD is affected by many factors.Psycho-cardiology intervention model has good short-term and long-term effects,and can enhance the sleep quality and improve the psy-chological status in CHD patients.
