高端人工晶状体在青光眼患者中应用的研究进展

由于患者对视力和脱镜需求不断提高以及技术的进步,高端人工晶状体(intraocular lens,IOL)包括老视矫正型IOL和散光矫正型IOL越来越多地被临床实践采用.研究表明部分青光眼患者植入高端IOL具有很高的脱镜率,视力效果佳,对比敏感度与健康眼相似,并且术后患者满意度高.然而目前尚缺乏足够的证据支持晚期青光眼患者植入高端IOL的安全性.当白内障摘除术与某些青光眼手术相结合时应谨慎选择高端IOL,因为可能有其他术中和术后因素影响术后的视觉质量.高端IOL在青光眼疑似患者、高眼压症患者和疾病早期控制良好的青光眼患者中往往有更好的结果.IOL的选择应根据患者期望的屈光结果、青光眼亚型、青光眼视野缺损类型和严重程度、青光眼手术类型来综合考虑.(国际眼科纵览,2024,48:1-7)
Research progress on the application of premium intraocular lenses in glaucoma patients
Due to the increasing demand for vision and spectacle independence among patients,as well as technological advancements,premium intraocular lenses(IOLs),including presbyopia correction IOLs and astigmatism correction IOLs,are increasingly being adopted in clinical practice.Existing studies have shown that some glaucoma patients implanted with premium IOLs have a high rate of spectacle inde-pendence,excellent visual acuity,and comparable contrast sensitivity to healthy subjects,with high postop-erative patient satisfaction.However,there is currently a lack of sufficient evidence to support the safety of-premium IOL implantation in patients with advanced glaucoma.When combining cataract extraction surgery with certain glaucoma surgeries,premium IOLs should be carefully selected,as there may be other intraoper-ative and postoperative factors that affect the visual quality after surgery.Premium IOL often has better re-sults in suspected glaucoma patients,patients with high intraocular pressure,and glaucoma patients with good early disease control.The selection of IOL should be comprehensively considered based on the patient's expected refractive outcome,glaucoma subtypes,the type and severity of glaucoma visual field defects,and the type of glaucoma surgery.(Int Rev Ophthalmol,2024,48:1-7)
