C2背根神经节脉冲射频联合三氧自体血疗法治疗老年颈源性头痛的效果

摘要
目的 探究C2背根神经节(DRG)脉冲射频微创技术(PRF)联合三氧自体血疗法(OAHT)治疗老年颈源性头痛(CEH)的效果.方法 选取2020年11月—2021年11月在吉林大学中日联谊医院收治的85例CEH患者进行回顾性分析,根据纳入排除标准,确定有效病例48例,根据治疗方案分为PRF组、PRF+OAHT组.PRF组共22例患者,行C2 DRG的PRF治疗;PRF+OAHT组共26例患者,行C2 DRG的PRF联合OAHT治疗,每日1次,疗程为1周.记录治疗前、治疗后1天、1周、1个月的视觉模拟量表(VAS)评分及治疗前、治疗后1个月的颈椎活动度(ROM)评分;采用临床总有效率评估临床效果,记录治疗期间的并发症及不良反应.结果 两组治疗后VAS评分均低于治疗前(P＜0.05),ROM评分低于治疗前(P＜0.05);治疗后1周、1个月PRF+OAHT组VAS评分低于PRF组(P＜0.05);两组治疗有效率均达90％以上,差异无统计学意义(P＞0.05).结论 PRF+OAHT组对老年CEH的缓解效果优于PRF组;两种方法均能减轻CEH引起的颈椎受限,且效果基本相同,治疗后均接近正常水平.
Abstract
Objective To investigate the efficacy of C2 dorsal root ganglion(DRG)pulsed radiofrequency(PRF)com-bined with ozonated autohemotherapy(OAHT)in the treatment of elderly patients with cervicogenic headache(CEH).Methods A total of eighty-five patients with CEH admitted to China-Japan Union Hospital of Jilin University from November 2020 to November 2021 were selected for review,and 48 effective cases were identified according to the inclusion and exclusion criteria and divided into PRF group and PRF+OAHT group according to the treatment plan.A total of 22 patients in the PRF group were treated with PRF for C2 DRG,and 26 patients in the PRF+OAHT group were treated with PRF for C2 DRG combined with OAHT once daily for 1 week.Visual analogue scale(VAS)was recorded before treatment,1 day,1 week and 1 month after treatment,and cervical range of mo-tion(ROM)score was recorded before treatment and 1 month after treatment;the overall clinical response rate was used to assess the clinical effect,and the complications and adverse reactions during treatment were recorded.Results After treatment,VAS score was lower than that before treatment(P＜0.05),ROM score was lower than that before treatment(P＜0.05);1 week and 1 month after treatment,VAS score in PRF+OAHT group was lower than that in PRF group(P＜0.05);the effective rate was more than 90％in both groups,and the difference was not statistically significant(P＞0.05).Conclusion The effect of PRF+OAHT group was better than PRF group in relieving senile CEH.Both methods could relieve cervical limitation caused by CEH,and the effect was basically the same,close to the normal level after treatment.
