摘要

目的 探讨微小RNA-21(miR-21)及叉头框蛋白M1(FOXM1)在妊娠期高血压(GHT)与子痫前期(PE)患者中的表达及意义.方法 选取妊娠期高血压疾病(HDP)患者150例,其中GHT患者112例作为GHT组、PE患者38例作为PE组,选择年龄及孕周匹配同期产检正常孕妇150例作为对照组;3组均于入组当天采集晨空腹外周静脉血4 mL,检测血清miR-21及FOXM1水平;分析GHT组及PE组患者血清miR-21与FOXM1 mRNA的相关性,绘制受试者工作特征曲线(ROC)计算曲线下面积(AUC),评估PE的影响因素;分析miR-21及FOXM1 mRNA对PE的预测价值及对妊娠结局的影响.结果 PE组、GHT组血清miR-21及FOXM1 mRNA水平低于对照组,PE组低于GHT组(P<0.05);PE组、GHT组miR-21与FOXM1 mRNA均呈正相关(P<0.05);miR-21及FOXM1 mRNA是PE的独立保护因素(P<0.05);基于miR-21及FOXM1 mRNA构建的风险模型评估PE的AUC值为0.841,高于miR-21及FOXM1 mRNA单独评估的AUC值(P<0.05);PE组不良妊娠结局发生率高于GHT组(x2=4.053,P=0.044),不良妊娠结局患者血清miR-21及FOXM1 mRNA水平低于正常妊娠者(P<0.05).结论 miR-21表达降低可能会靶向下调FOXM1 mRNA表达,促进GHT与PE的发生、发展,且与不良妊娠结局有关.

Abstract

Objective To investigate the expression and significance of microRNA-21(miR-21)and Forkhead Box Protein M1(FOXM1)in patients with Gestational Hypertension(GHT)and Preeclampsia(PE).Methods A total of 150 patients with hypertensive disorders of pregnancy(HDP)were enrolled,comprising 112 in the GHT group and 38 in the PE group,alongside 150 control subjects matched by age and gestational age.On the day of enrollment,4 mL of fasting peripheral venous blood was collected from all participants to measure serum miR-21 and FOXM1 levels.The correlation between serum miR-21 and FOXM1 mRNA in the GHT and PE groups were analyzed,the receiver operating characteristic(ROC)curve was drew,and the area under the curve(AUC)to evaluate factors influencing PE was calculated.It were also assessed the predictive value of miR-21 and FOXM1 mRNA for PE and their impact on pregnancy outcomes.Results Serum miR-21 and FOXM1 mRNA levels were significantly lower in the PE and GHT groups compared to controls,with the lowest levels observed in the PE group(P<0.05).Both groups showed a positive correlation between miR-21 and FOXM1 mRNA(P<0.05).MiR-21 and FOXM1 mRNA served as independent protective factors against PE(P<0.05).A risk model based on miR-21 and FOXM1 mRNA yielded an AUC of 0.841 for assessing PE,surpassing the AUCs when assessed individually(P<0.05).The PE group exhibited a higher incidence of adverse pregnancy outcomes compared to the GHT group(x2=4.053,P=0.044),with affected patients showing lower serum levels of miR-21 and FOXM1 mRNA than those with normal outcomes(P<0.05).Conclusion Decreased miR-21 expression may lead to the downregulation of FOXM1 mRNA,contributing to the development of GHT and PE and increasing the risk of adverse pregnancy outcomes.
