摘要
目的 探讨子宫内膜癌组织中sall4和上皮间质转化标志物[波形蛋白(vimentin)和钙黏蛋白(e-cadherin)]的表达及对肿瘤生物学行为的影响.方法 采用免疫组化方法检测子宫内膜癌、子宫内膜组织及增生子宫内膜组织中sall4、vimentin和E-cadherin的阳性表达率;采用spearman相关分析子宫内膜癌中sall4与vi-mentin和E-cadherin表达的相关性,logistis回归分析考察相关临床病理因素的预后风险,Kaplan-Meier法分析sall4阳性表达患者术后3年生存率.结果 子宫内膜癌组织中sall4和vimentin的阳性表达率高于子宫内膜组织和增生子宫内膜组织、E-cadherin的阳性表达率,低于子宫内膜组织和增生子宫内膜组织,差异有统计学意义(P<0.05);子宫内膜癌组织中sall4、E-cadherin阳性表达与肿瘤浸润程度(χ2=4.286、8.148)、肿瘤组织分化(χ2=6.166、6.834)、肿瘤直径(χ2=4.115、6.074)、FIGO分期(χ2=5.287、6.163)及淋巴结侵犯(χ2=4.473、4.463)有关(P<0.05),而与年龄(χ2=0.051、0.003)无关(P>0.05);vimentin阳性表达与肿瘤浸润程、肿瘤组织分化、肿瘤直径及FIGO分期有关(χ2=6.787、7.577、5.370、4.450,P<0.05),而与淋巴结侵犯、年龄无关联(χ2=0.617、0.207,P>0.05);sall4与E-cadherin在子宫内膜癌组织中的表达存在负相关(r=-0.328,P<0.05),与vimentin表达存在正相关(r=0.396,P<0.01);手术后子宫内膜癌患者影响预后的风险因素有肿瘤组织分化及浸润程度、淋巴结侵犯、FIGO分期及sall阳性(β=1.662、1.341、2.277、1.895、1.401,P<0.05),sall4高表达患者术后3年生存率明显低于低表达患者生存率(P<0.05).结论 sall4﹑vimentin及E-cadherin在子宫内膜癌中的异常表达与肿瘤发生和进展存在关联.
Abstract
Objective To investigate the expression of sall4 and epithelial-mesenchymal transition ( EMT) markers and the influence of its biological behavior in endometrial carcinoma ( EC) . Methods The positive expressions of sall4,vimentin,and E-cadherin in EC,endometrial hyperplasia and endometrial tissues were assessed by immunohistochemisty. Spearman correlation analysis was used to test the correlation between sall4 and markers of EMT. logistic model was used to investigate the prognostic risk of relevant clinicopathological factors in EC. Kaplan-Meier survival curve analysis survival curves for positive and negative expression of sall4. Results The expressions of sall4 and vimentin in EC were higher than those in endometrial tissues and endometrial hyperplasia . The expression of E-cadherin in EC was lower than those in endometrial tissues and endometrial hyperplasia. The expressions of sall4 and E-cadherin were positively correlated with the degree of depth of invasion (x2=4 . 286,8 . 148 ),tumor histological grade (x2=6 . 166,6 . 834 ),lymph node metastasis (x2=4. 473,4. 463),tumor diameter(x2=4. 115,6. 074) and surgial-pathological stage (x2=5. 287,6. 163;P<0.05),which were not correlated with patients'age (x2=0.051,0.003;P>0.05 ) . Vimentin expression was positively correlated with the degree of depth of invasion (x2=6. 787 ),tumor histological grade (x2=7 . 577 ),tumor diameter (x2=5 . 370 ) and surgial-pathological stage (x2=4. 450,P<0.05),which were not correlated with patients'age (x2=0.207) and lymph node metastasis (x2=0.617,P>0.05). The expression of sall4 was negatively correlated with the expression of E-cadherin (rs=-0.328,P<0.05) and was positively correlated with the expression of vimentin in EC ( r=0.396,P<0.01 ) . The degree of depth of invasion (β=1 . 662 ),tumor histological grade (β=1 . 341 ),lymph node metastasis (β=2 . 277 ),the positive expression of sall4 (β=1 . 401 ),and surgial-pathological stage (β=1 . 895 ) was a risk factor for prognosis in EC patients with surgical treatment ( P<0.05 ) . The survival rate of EC patients with high expression of sall43 years after surgery was lower than patients with negative expression ( P<0.05 ) . Conclusion Sall4,vimentin,and E-cadherin is abnormal expression of EC,and it is closely correlated with occurrence and development of EC.
