摘要
目的:通过探讨系统化营养干预对青海地区晚期胃癌二线化疗患者化疗疗效、不良反应发生情况、营养状况及生活质量的影响,为青海地区晚期胃癌患者实施营养干预提供理论支持.方法:收集2020年1月1日—2023年1月1日青海大学附属医院收治一线化疗失败的晚期胃癌患者200例,采用随机数表法分为营养干预组(n=100)和非营养干预组(n=100).营养干预组实施系统化营养干预及护理,非营养干预组接受常规治疗,比较两组患者的近期疗效、化疗不良反应发生率、营养状况和生活质量.结果:①营养干预组治疗4周期后临床获益率(CBR)为79.00%,高于非营养干预组治疗4周期后CBR(66.00%)(P<0.05).②营养干预组治疗期间恶心、呕吐、腹泻、疲乏发生率较非营养干预组降低(P<0.05),但两组患者便秘发生率比较,差异无统计学意义(P>0.05).③营养干预组治疗4周期后体质量指数(BMI)、血红蛋白(Hb)、白蛋白(ALB)、总蛋白(TP)、淋巴细胞计数(TLC)水平均高于非营养干预组(P<0.05).④营养干预组治疗4周期后躯体功能、角色功能、情绪功能、认知功能、社会功能评分和总体健康评分均高于非营养干预组(P<0.05).结论:系统化营养干预通过改善青海地区晚期胃癌二线化疗患者的营养状况,提高其化疗耐受性,降低化疗相关不良反应发生率,对改善晚期胃癌患者生活质量具有积极意义.
Abstract
Objective:By exploring the effects of systematic nutritional intervention on chemotherapy efficacy,incidence of adverse reactions,nutritional status and quality of life in patients with advanced gastric cancer treated with second-line chemotherapy,this study aims to provide theoretical support for the implementation of nutritional intervention in patients with advanced gastric cancer in Qinghai.Methods:Colect 200 advanced gastirc cancer patients who failed chemotherapy at the first line of Qinghai Univevrsity Affiliated Hospital from Januarylst,2020 to Januarylst,2023,and use a random number table method to divide them into a nutritional intervention group(n=100)and a non-nutritional intervention group(n=100).the nutritional intervention group implemented systematic nutritional interventions and nursing care,and the non-nutritional intervention group received conventional treatment,and the two groups were compared with respect to the efficacy of chemotherapy,the incidence rate of adverse effects of chemotherapy,the nutritional status,and the score of the quality of life.Results:① The clinical benefit rate(CBR)after 4 cycles of treatment in the nutritional intervention group was 79.00%comparing to 66.00%after 4 cycles of treatment in the non-nutritional intervention group,and the difference was statistically significant(P<0.05).② The incidence of nausea,vomiting,diarrhea and fatigue in the nutrition intervention group was significantly lower than that in the non-nutrition intervention group,with statistical significance(P<0.05).There was no significant difference in the incidence of constipation between the two groups(P>0.05).③ The body mass index(BMI),hemoglobin(Hb),albumin(ALB),total protein(TP),and lymphocyte count(TLC)in the nutritional intervention group were higher than the values of each nutritional index in the non-nutritional intervention group after 4 cycles of treatment,and the difference was statistically significant(P<0.05).④ Somatic functioning,role functioning,emotional functioning,cognitive functioning,social functioning and general health scores were higher in the nutritional intervention group than in the non-nutritional intervention group after 4 cycles of treatment,and the difference was statistically significant(P<0.05).Conclusion:Systematic nutrition intervention has positive significance for improving the quality of life of advanced gastric cancer patients by improving the nutritional status of patients with second-line chemotherapy in Qinghai province,improving chemotherapy tolerance and reducing the occurrence of chemotherapy-related toxic side effects.
