目的:系统评价小青龙汤合三子养亲汤加减治疗小儿咳嗽变异性哮喘的临床疗效和安全性,为临床用药提供循证医学依据.方法:通过检索中国知网、万方医学数据库、维普数据库、中国生物医学文献服务系统、PubMed数据库、Cochrane Library数据库、Web of science数据库,检索时间均从建库至2023 年07 月01 日,收集小青龙汤合三子养亲汤加减治疗小儿咳嗽变异性哮喘的随机对照试验研究,提取数据并应用Rev Man 5.3 软件进行Meta分析,TSA 0.9 软件进行试验序贯分析.结果:共纳入16 篇随机对照试验研究,1412 例患者(其中试验组706 例,对照组706 例).Meta分析显示:试验组临床总有效率(OR=4.65,95%CI[3.24,6.66],Z=8.36,P<0.00001)、第 1 秒用力呼气量(FEV1)(MD=0.62,95%CI[0.30,0.94],Z=3.77,P=0.0002)、用力肺活量(FVC)(MD=0.61,95%CI[0.30,0.92],Z=3.84,P=0.0001)、呼气峰流速百分比(PEF%)(MD=7.77,95%CI[6.15,9.39],Z=9.39,P<0.00001)均优于对照组,差异均有统计学意义,此外,试验序贯分析提示结果可靠.结论:与单纯西药治疗比较,小青龙汤合三子养亲汤加减单用或联用西药治疗小儿咳嗽变异性哮喘,可提高临床疗效,改善肺功能和临床症状,且安全性较好.
Meta analysis and trial sequential analysis of modified Xiaoqinglong Decoction combined with Sanziyangqin Decoction in the treatment of cough variant asthma in children
This paper systematically evaluates the clinical efficacy and safety of the modified Xiaoqinglong Decoc-tion combined with Sanziyangqin Decoction in the treatment of cough variant asthma in children,so as to provide ev-idence-based medical basis for clinical medication.By searching CNKI,Wanfang Medical Database,VIP Database,China Biomedical Literature Service System,PubMed,Cochrane Library and Web of Science(from database con-struction July 01,2023),randomized controlled studies on modified Xiaoqinglong Decoction combined with Sanziy-angqin Decoction in the treatment of cough variant asthma in children were collected.Meta analysis was conducted by Rev Man 5.3 software and trial sequential analysis was conducted by TSA 0.9 software.A total of 16 RCT arti-cles were obtained,including 1412 patients,among which 706 were in the test group and 706 were in the control group.Meta analysis showed that the total clinical response rate(OR=4.65,95%CI[3.24,6.66],Z=8.36,P<0.00001),the forced expiratory volume in second 1(FEV 1)(MD=0.62,95%CI[0.30,0.94],Z=3.77,P=0.0002),the forced vital capacity(FVC)(MD=0.61,95%CI[0.30,0.92],Z=3.84,P=0.0001),and the percentage of peak expiratory flow rate(PEF%)(MD=7.77,95%CI[6.15,9.39],Z=9.39,P<0.00001)of the test group were better than those of the control group,and all the differences were statistically significant.Be-sides,trial sequential analysis suggested reliable results.In conclusion,compared with western medicine,modified Xiaoqinglong Decoction combined with Sanziyangqin Decoction can improve clinical efficacy,lung function and clin-ical symptoms in the treatment of cough variant asthma in children with high safety.
