目的:本研究拟对急性ST段抬高型心肌梗死(STEMI)患者外周血单核细胞中miR-23a和27a表达水平及PCI术后通过血管内超声测定冠脉罪犯斑块相关参数之间的相关性进行分析,为STEMI患者的诊断和治疗提供新靶点.方法:本研究共纳入STEMI患者95例(其中92例行血管内超声检查)、对照组30例,通过测定其外周血单核细胞中miR-23a和27a表达水平,及STEMI患者PCI术后通过IVUS测定冠脉罪犯斑块相关参数,对两者之间的相关性进行分析.结果:STEMI患者外周血单核细胞中miR-23a和miR-27a的表达水平显著高于对照组(7.12±3.82比1.00±0.23,P<0.000 1;10.25±4.68 比1.00±0.33,P<0.000 1);STEMI患者外周血单核细胞中miR-23a和miR-27a的表达水平与最小管腔面积负相关(r=-0.899 1,P<0.01和r=-0.904 3,P<0.01)、与斑块负荷正相关(r=0.894 4,P<0.01和r=0.917 4,P<0.01),TCFA组显著高于无TCFA组(8.35±2.19比5.23±1.84,P<0.000 1和12.33±2.16比7.38±2.49,P<0.000 1).结论:STEMI患者外周血单核细胞中miR-27a和miR-23a表达水平及PCI术中通过血管内超声测定冠脉罪犯斑块相关参数之间显著相关,最小管腔面积越小、斑块负荷越重、TCFA越明显的患者,其外周血单核细胞miR-27a和miR-23a表达水平越高.这为STEMI患者的早期诊断提供新的标记物、为介入诊疗提供策略支持并为患者的长期治疗提供新靶点.
Relationship between miR-27a and miR-23a expression levels in peripheral blood monocytes and coronary artery culprit plaques in patients with STEMI
Objective:This study aims to analyze the correlation between miR-23a and 27A expression levels in peripheral blood mononuclear cell of STEMI patients and parameters related to coronary culprit plaques measured by IVUS after PCI,in order to provide new targets for the diagnosis and treatment of STEMI patients.Methods:A total of 95 STEMI patients(92 IVUS)and 30 controls were enrolled in this study,miR-23a and 27A expression levels in peripheral blood mononuclear cell of both groups were tested and the correlation between miR-23a and 27A expression levels of STEMI patients and parameters related to coronary culprit plaques measured by IVUS after PCI were analyzed.Results:The expression levels of miR-23a and miR-27a in peripheral blood monocytes of STEMI patients were significantly higher than those of the control group(7.12±3.82 vs 1.00±0.23,P<0.000 1;10.25±4.68 vs 1.00±0.33,P<0.000 1);moreover,the expression levels of miR-23a and miR-27a in peripheral blood monocytes of STEMI patients were negatively correlated with minimal lumen area(r=-0.899 1,P<0.01;r=-0.904 3,P<0.01)and positively correlated with plaque burden(r=0.894 4,P<0.01;r=0.917 4,P<0.01).The expression levels of miR-23a and miR-27a in TCFA group were significantly higher than those in non-TCFA group(8.35±2.19 vs 5.23±1.84,P<0.000 1;12.33±2.16 vs 7.38±2.49,P<0.000 1).Conclusion:The expression levels of miR-27a and miR-23a in peripheral blood mononuclear cells of patients and PCI intraoperative by IVUS determination of significant correlation between coronary culprit plaque related parameters,patients with smaller minimal lumen area,heavier plaque burden and more obvious TCFA had higher expression levels of miR-27a and miR-23a in peripheral blood monocytes.This study provides new markers for early diagnosis of STEMI patients,strategic support for interventional diagnosis and treatment,and new targets for long-term treatment of patients.
