摘要
[目的]探讨糖尿病视网膜病变(diabetic retinopathy,DR)与生辰五运六气的相关性.[方法]通过调查问卷方式,收集2019年5月22日至2021年9月13日在广州中医药大学第一附属医院、广东省第二中医院门诊就诊或住院的1 227例糖尿病(diabetes mellitus,DM)患者的一般资料和五运六气资料,通过单因素分析及多因素Logistic回归分析,探讨DR与一般临床资料及生辰五运六气的相关性.[结果](1)一般资料方面,与单纯DM组比较,DR组患者的尿酸(UA)、血肌酐(Scr)、尿微量白蛋白/尿肌酐(UACR)水平显著升高,差异均有统计学意义(P＜0.05或P＜0.01).(2)五运六气资料方面,DR患者的生辰年运以土运偏多,生辰主气以阳明燥金、太阴湿土偏多,生辰客气以太阴湿土偏多;与单纯DM组比较,DR组患者的生辰司天之气/在泉之气为少阳相火/厥阴风木所占比例显著升高,少阴君火/阳明燥金所占比例显著降低,差异均有统计学意义(P＜0.05或P＜0.01).(3)多因素Logistic 回归分析结果显示,DM患者生辰司天之气/在泉之气为少阴君火/阳明燥金为DR的独立保护因素,Scr、UACR为DR发生的独立危险因素,差异均有统计学意义(P＜0.05或P＜0.01).[结论]DM患者的生辰五运六气特征对其是否合并DR有一定影响;DR患者的生辰五运六气的五行特征以木、土、金偏多.
Abstract
Objective To explore the correlation between diabetic retinopathy(DR)and five circuits and six qi of birthdate.Methods A questionnaire survey was carried out.The general clinical data and the birthdate five-circuit and six-qi data were collected from 1 227 patients with diabetes mellitus(DM)who were treated in clinic or hospitalized in the First Affiliated Hospital of Guangzhou University of Chinese Medicine and the Guangdong Second Traditional Chinese Medicine Hospital from May 22,2019 to September 13,2021.Univariate analysis and multivariate logistic regression analysis were performed to explore the correlation of DR with the general clinical data and the birthdate five-circuit and six-qi data.Results(1)The analysis of the general clinial data showed that DR patients had higher levels of uric acid(UA),serum creatinine(Scr)and urinary albumin-to-creatinine ratio(UACR)than the patients with simple DM,and the differences were statistically significant(P＜0.05 or P＜0.01).(2)The analysis of the birthdate five-circuit and six-qi data showed that DR patients were usually born in the year of earth phase with the dominant qi being yangming dry gold or taiyin damp earth,and with the guest qi being taiyin damp earth.Compared with the simple DM group,the proportion of sitian-zaiquan semiannual circuit qi being shaoyang ministerial fire-jueyin wind wood in the DR group was significantly increased,and the proportion of sitian-zaiquan semiannual circuit qi being shaoyin monarch fire-yangming dry gold was significantly decreased,the differences being statistically significant(P＜0.05 or P＜0.01).(3)The results of multivariate logistic regression analysis showed that the sitian-zaiquan semiannual circuit qi being shaoyin monarch fire-yangming dry gold in DM patients was the independent protective factor for DR,and Scr and UACR were the independent risk factors for DR,with statistically significant differences(P＜0.05 or P＜0.01).Conclusion The features of five circuits and six qi of birthdate in DM patients are correlated with the complication of DR to some extend;the DR patients are usually have the birthdate five circuits characterized by wood,earth and gold.
