益肝明目颗粒对糖尿病视网膜水肿大鼠组织形态学及VEGF、ICAM-1表达的影响

目的:通过观察益肝明目颗粒对糖尿病视网膜水肿大鼠血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)、细胞间黏附分子-1(intercellular cell adhesion molecule-1,ICAM-1)蛋白表达水平的影响,探讨其改善糖尿病视网膜水肿的机制。方法:将48只大鼠采用随机分为正常组(8只)和造模组(40只)。造模组进行糖尿病视网膜水肿造模;造糖尿病模成功后4周、8周,对正常组和造模组进行眼底血管荧光造影,对比检测造模是否成功。将造模成功的剩余38只大鼠随机分为模型组(7只)、阳性药物组(7只)及益肝明目颗粒低、中、高剂量组,每组8只;加上正常组剩余大鼠6只,共计6组。正常组和模型组予以生理盐水灌胃;阳性药物组予以灌胃安多明0。09 g/kg,益肝明目颗粒低、中、高剂量组分别以2。84、5。68、11。36 g/kg益肝明目颗粒剂灌胃。治疗持续4周后,采集视网膜标本,HE染色观察视网膜组织组织形态学,光学相干断层扫描(optical coherence tomography,OCT)观察视网膜厚度,Western blot分别检测视网膜组织VEGF、ICAM-1的表达水平,ELISA法分别检测血清VEGF、ICAM-1的表达水平蛋白表达水平。结果:与模型组对比,治疗组的视网膜结构有明显差异;阳性药物组、中药组视网膜厚度均明显减少(P<0。01);阳性药物组、中药组的血清ICAM-1、VEGF水平均明显降低(P<0。01);中药组的ICAM-1水平降低(P<0。05),VEGF水平明显降低(P<0。01)。与阳性药物组相比,YGMMG低剂量组、中剂量组视网膜厚度均明显减少(P<0。01);与阳性药物组、YGMMG低剂量组、高剂量组相比,中剂量组血清ICAM-1、VEGF水平明显降低(P<0。01);与阳性药物组相比,中剂量组的ICAM-1蛋白水平明显降低(P<0。01)。与YGMMG高剂量组视网膜厚度比较,YGMMG中剂量视网膜厚度减少,差异具有统计学意义(P<0。05)。与低剂量组对比,中剂量组的VEGF、ICAM-1蛋白表达水平降低(P<0。05)。结论:益肝明目颗粒能有效降低糖尿病视网膜水肿程度、改善视网膜组织形态,可能与下调ICAM-1、VEGF的表达水平有关。
Effect of Yigan Mingmu granules on histomorphology and expression of VEGF and ICAM-1 in diabetes retinal edema rats
Objective:By observing the effect of Yigan Mingmu Granules on the protein expression levels of vascular endothe-lial growth factor(VEGF)and intercellular cell adhesion molecule-1(ICAM-1)in diabetes rats with retinal edema,the mecha-nism of improving diabetes retinal edema was explored.Methods:48 rats were randomly divided into a normal group(8 rats)and a model group(40 rats).Diabetes retinal edema model was established in the model group;At 4 and 8 weeks after the successful establishment of the diabetes model,fundus fluorescein angiography was performed in the normal group and the model group to compare whether the model was successful.The remaining 38 rats that were successfully modeled were randomly divided into model group(7 rats),positive drug group(7 rats),and low,medium,and high dose groups of Yigan Mingmu Granules,with 8 rats in each group;Add 6 remaining rats from the normal group,totaling 6 groups.The normal group and model group were given physiological saline by gavage;The positive drug group was gavaged with 0.09 g/kg Andomin,while the low,medium,and high dose groups of Yigan Mingmu Tang were gavaged with 2.84 g/kg,5.68 g/kg,and 11.36 g/kg Yigan Mingmu Granules,respec-tively.After 4 weeks of treatment,retinal specimens were collected for HE staining to observe the morphology of retinal tissue,optical coherence tomography(OCT)to observe retinal thickness,Western blot to detect the expression levels of VEGF and ICAM-1 in retinal tissue,and ELISA to detect the expression levels of serum VEGF and ICAM-1 protein.Results:Compared with the model group,there was a significant difference in the retinal structure of the treatment group;The retinal thickness of both the positive drug group and the traditional Chinese medicine group decreased significantly(P<0.01);The serum ICAM-1 and VEGF levels in the positive drug group and the traditional Chinese medicine group were significantly reduced(P<0.01);The level of ICAM-1 in the traditional Chinese medicine group decreased(P<0.05),while the level of VEGF significantly decreased(P<0.01).Compared with the positive drug group,the retinal thickness of the low-dose and medium dose YGMMG groups was significantly reduced(P<0.01);Compared with the positive drug group,YGMMG low-dose group,and high-dose group,the se-rum ICAM-1 and VEGF levels in the medium dose group were significantly reduced(P<0.01);Compared with the positive drug group,the ICAM-1 protein level in the medium dose group was significantly reduced(P<0.01).Compared with the high-dose group of YGMMG,the moderate dose group of YGMMG showed a statistically significant decrease in retinal thickness(P<0.05).Compared with the low-dose group,the expression levels of VEGF and ICAM-1 proteins in the medium dose group de-creased(P<0.05).Conclusion:Yigan Mingmu Granules can effectively reduce the degree of retinal edema in diabetes and im-prove the retinal tissue morphology,which may be related to the down-regulation of ICAM-1 and VEGF expression levels.
