Clinical Retrospective Study on the Effect of Polymyxin B Loading Dose on Renal Function
OBJECTIVE Exploring the effect of clinical dosage of polymyxin B on renal function.METHODS From January 1,2019 to April 17,2021,a total of 469 patients treated with injection polymyxin B sulfate in the one hospital of Nanchang University were retrospectively analyzed.They were divided into unused loading dose groups ac-cording to the different doses used.133 cases(group A),336 cases using loading dose group(group B).The chan-ges in renal function,combined use of nephrotoxic drugs,combined use of other antibacterial drugs,and acute kidney injury were compared between the two groups of patients before and after medication,and their correlations were ana-lyzed.RESULTS The abnormal rate of creatinine in group B was 24.78%,that of group A was 19.35%(P>0.05),that of Blood urea nitrogen in group B was 30.21%,and that of group A was 19.57%(P>0.05).There was no statistical significance between the two groups.Combined use of nephrotoxic drugs(P =0.043,OR =0.317)was an independent risk factor for acute kidney injury.CONCLUSION The use of polymyxin B loading dose in this study has no effect on renal function,and even the overload dose does not increase the incidence of acute kidney inju-ry.Therefore,it further proves that dose adjustment does not need to be based on different degrees of renal function.However,when combined with nephrotoxic drugs,it is necessary to be vigilant about the occurrence of acute kidney injury.
