2020~2022年某院泌尿外科患者尿培养病原菌分布及药敏分析

目的 分析近3年深圳市坪山区人民医院泌尿外科患者尿培养病原菌分布及药敏结果,为临床合理选用抗菌药物优化治疗方案提供参考.方法 收集2020~2022年我院泌尿外科患者尿培养中分离的非重复病原菌及药敏结果,回顾性分析尿培养的阳性率,病原菌分布特点、变迁情况及耐药性变化.结果 送检的10163份标本中,检出974株非重复病原菌,检出率为9.58%.其中革兰阴性菌763株,占78.34%;革兰阳性菌171株,占17.56%;真菌40株,占4.11%.革兰阴性菌前三位为大肠埃希菌(60.47%)、奇异变形菌(6.67%)、肺炎克雷伯菌(4.72%);革兰阳性菌以粪肠球菌(6.16%)为主.药敏结果显示:大肠埃希菌对氨苄西林、复方新诺明、头孢曲松、环丙沙星、左氧氣沙星的耐药率呈逐年上升趋势,2022年耐药率均>30%;对哌拉西林/他唑巴坦、头孢吡肟、头孢替坦、呋喃妥因、阿米卡星、妥布霉素敏感性较高,耐药率均<10%,且未检出对碳青霉烯类耐药的大肠埃希菌.粪肠球菌对四环素、红霉素耐药率高(均>70%),且对红霉素的耐药率呈逐年上升趋势;对氨苄西林、青霉素、万古霉素敏感性高,耐药率均<20%,且未检出对呋喃妥因、利奈唑胺、替加环素耐药的粪肠球菌.结论 我院泌尿外科患者尿培养病原菌仍以大肠埃希菌为主,经验治疗可根据近年耐药情况,结合患者病情、用药史,以及抗菌药物代谢特点,合理选用抗菌药物;待药敏结果回报后,结合经验用药治疗效果,调整给药方案,提高疗效,减少不良反应及耐药菌产生.
Distribution and Drug Sensitivity Analysis of Pathogen in Urinary Culture of Patients in Urology Department of One Hospital from 2020 to 2022
OBJECTIVE To analyze the pathogen distribution and drug sensitivity of urinary culture in patients of urology department in Pingshan District People's Hospital of Shenzhen in recent 3 years,and to provide reference for clinical rational antibiotic selection and optimization of treatment.METHODS From 2020 to 2022,non-dupli-cate pathogen from urine culture of urological patients in our hospital and the results of drug sensitivity test were col-lected.The positive rate of urine culture,the distribution characteristics and changes of pathogen and the changes of drug resistance were retrospectively analyzed.RESULTS Among 10163 samples,974 strains of non-repeated patho-gens were detected,with the detection rate of 9.58%.Among the 974 specimens,763 strains of Gram-negative bacte-ria accounted for 78.34%;171 strains of Gram-positive bacteria accounted for 17.56%;40 strains of fungi accounted for 4.11%.The top three gram-negative bacteria were Escherichia coli(60.47%),Proteus Mirabilis(6.67%)and Klebsiella pneumoniae(4.72%).The gram-positive bacteria were mainly Enterococcus faecalis(6.16%).The drug sensitivity test showed that the drug resistance rates of Escherichia coli to ampicillin,cotrimoxazole,ceftriaxone,cipro-floxacin and levofloxacin increased year by year,and the drug resistance rates were all over 30%in 2022.The sensi-tivity rate of Escherichia coli to piperacillin/tazobactam,cefepime,cefotetan,furantoin,amicacin and tobramycin was high,and the drug resistance rates were all lower than 10%.No carbapenems resistant Escherichia coli was detec-ted.The drug resistance rate of Enterococcus faecalis to tetracycline and erythromycin was high(both>70%),and the resistance rate to erythromycin showed an increasing trend year by year.The drug sensitivity rate of Enterococcus faecalis to ampicillin,penicillin and vancomycin was high,and the drug resistance rates were all lower than 20%.No Enterococcus faecalis resistant to furantoin,linezol id and tigacycline were detected.CONCLUSION Escherichia coli is still the main pathogen in urinary culture of patients in urology department of our hospital.Rational antimicrobial drugs should be selected according to the drug resistance situation in recent years,the patient's clinical manifesta-tions,previous medication use,and the characteristics of antibiotic pharmacokinetics.rational selection of antibiotics.Then,according to the results of drug sensitivity test,combined with the therapeutic effect of empirical drug use,the administration plan was adjusted to improve the therapeutic efficacy and reduce adverse reactions and the generation of drug-resistant bacteria.
