目的 探讨活血化瘀药对不稳定型心绞痛(Unstable angina,UA)患者纤溶酶原激活物抑制物(Plasminogen activator inhibitor-l,PAI-1)、组织型纤溶酶原激活剂(Tissue-type plasminogen activator,t-PA)水平的影响,进而评价活血化瘀药对不稳定型心绞痛患者临床疗效,从而为临床预防治疗不稳定型心绞痛提供依据.方法 通过检索PubMed、EMbase、The Cochrane Library、Web of Science、ClinicalTrials.gov、万方数据和中国知网数据库,收集活血化瘀药联合西医常规治疗UA的随机对照试验(RCT)文献,采用RevMan 5.4软件进行Meta分析.结果 共纳入22项研究,涉及1998例研究对象.Meta分析结果显示,与西药相比,联用活血化瘀中药可以进一步降低UA患者血清PAI-1水平[SMD=-1.96,95％CI(-2.50,-1.43)],Z=7.18,P＜0.00001,升高 UA 患者 t-PA 水平[SMD=0.55,95％CI(0.41,0.69)],Z=7.85,P＜0.00001.结论 与常规西药治疗相比,联用活血化瘀药可进一步降低UA患者体内PAI-1水平、升高t-PA水平,改善患者纤溶活性.
Effect of Blood-activating/Stasis-resolving Chinese Herbal Drugs on PAI-1 and t-PA in Patients with Unstable Angina:a Meta-analysis
OBJECTIVE To explore the effect of blood-activating/stasis-resolving Chinese herbal drugs on the levels of PAI-1 and t-PA in patients with unstable angina pectoris(UA),and to evaluate the clinical efficacy of blood-activating/stasis-resolving Chinese herbal drugs on UA,to provide references for the treatment strategy of UA pa-tients.METHODS PubMed,EMbase,The Cochrane Library,Web of Science,ClinicalTrials.gov,WanFang Data and CNKI databases were electronically searched to collect randomized controlled trials(RCTs)that reported blood-activating/stasis-resolving Chinese herbal drugs therapy for UA combined with conventional Western treatment.Meta-analysis was then performed using RevMan 5.4 software.RESULTS A total of 22 studies were included,involving 1998 study subjects.Meta analysis results showed that compared with conventional Western medicine,the combina-tion of traditional Chinese medicine for promoting blood circulation and resolving stasis can reduce serum PAI-1 lev-els in UA patients[SMD=-1.96,95％CI(-2.50,-1.43)],Z=7.18,P＜0.00001,and increase t-PA levels in UA patients[SMD=0.55,95％CI(0.41,0.69)],Z=7.85,P＜0.00001.CONCLUSION Compared with con-ventional Western treatment,the combination of activating blood circulation and removing blood stasis can further re-duce the level of PAI-1 in UA patients,increase the level of t-PA,improve the fibrinolytic activity of patients.
