摘要

目的 探讨FOCUS-PDCA管理工具在降低自动化静脉用药调配中心(PIVAS)差错率的应用及效果.方法 以2022年1~12月自动化设备运行下PIVAS所产生的相关差错为研究背景,依照FOCUS-PDCA管理模式,通过成立质量管理小组,借助绘制流程图、柏拉图、鱼骨图等工具找出差错原因,拟定改进方案,以降低差错率为目标.统计2023年1~12月干预后各环节差错率,评价管理成效.结果 实施FOCUS-PDCA循环管理后,总差错率由0.48‰降为0.23‰,降幅52.08%.摆药差错由0.21%降为0.08‰,贴签差错由0.14‰降为0.08‰,分拣差错由0.07‰降为0.03‰,均有统计学差异(P<0.05).结论 FOCUS-PDCA管理工具对降低自动化PIVAS的差错率成效显著,有助于提高输液质量安全,为进一步规范自动化智能化静脉调配中心的建设提供依据.

Abstract

OBJECTIVE To explore the application and effectiveness of FOCUS-PDCA management tool on re-ducing the error rate of automatic Pharmacy Intravenous admixture Service(PIVAS).METHODS Based on the re-search background of PIVAS errors from January to December 2022 with the application of automatic equipment,a quality control group was set up according to the FOCUS-PDCA management model to find out the causes of errors with the help of flowchart,plato,fishbone diagram and other tools,and an improvement plan was drawn up to reduce the error rate.The error rate of each link after intervention was measured from January to December 2023,and the management effectiveness was evaluated.RESULTS After the implementation of FOCUS-PDCA cycle,the total er-ror rate decreased from 0.48‰ to 0.23‰,a decrease of 52.08%.Dispensing error decreased from 0.21‰ to 0.08‰,labeling error decreased from 0.14‰ to 0.08‰,and sorting error decreased from 0.07‰ to 0.03‰,all of them have statistical significance(P<0.05).CONCLUSION The FOCUS-PDCA management tool is effective in reducing the error rate of automatic PIVAS,which is helpful to improve the quality and safety of infusion,and pro-vides a basis for further standardizing the construction of automatic Pharmacy Intravenous admixture Service.
