Application of academic thoughts of Yan-Zhao physicians in the treatment of acne vulgaris
Acne vulgaris(AV)is one of the most common dermatological diseases, causing mental stress and economic burden to patients due to its lingering complications.Traditional Chinese medicine(TCM)has demonstrated strong advantages in the treatment of AV.This article mainly elab-orates on the understanding and treatment ideas of the etiology and pathogenesis of AV from the He-jian School,Yishui School, and Zhongxi Huitong School:The Hejian School, represented by Liu Wansu, advocates the"heat theory,"believing that the accumulation of heat in the lung meridian com-bined with the invasion of wind pathogens leads to stagnation of qi and blood and blockage of facial me-ridians,resulting in lung meridian wind-heat type AV.The treatment principle is to use cold and pun-gent herbs to clear heat and resolve stagnation. The Yishui School, with Li Dongyuan as its core fig-ure,proposes treating various diseases from the perspective of"treating the spleen and stomach."It believes that AV is often caused by excessive consumption of spicy and greasy foods, leading to inter-nal damp-heat in the spleen and stomach, which manifests on the face. Therefore, the key to treat-ment is to strengthen the spleen and promote yang. The Huitong School, represented by Zhang Xi-chun,integrates Chinese and Western medicine and is adept at using heat-clearing and detoxifying,blood-activating,and stasis-dissolving drugs to treat carbuncles. It has significant clinical implications for treating the phlegm-stasis type AV,which is caused by the prolongation of pathogenic factors and the transformation of heat into phlegm, leading to the stagnation of phlegm and blood in the skin.
