肾经水肿汤对糖尿病肾病大鼠的保护作用及调节机制

目的:探讨肾经水肿汤(Shenjingshuizhong decoction)对糖尿病肾病(DN)大鼠的保护作用及调节机制。方法:将60 只SPF级SD雄性大鼠随机分为正常对照组、DN 模型组、肾经水肿汤低剂量组(1g/kg)、肾经水肿汤中剂量组(2g/kg)、肾经水肿汤高剂量组(4g/kg)与阳性对照组(α-硫辛酸,60mg/kg),每组10 只。其中,DN模型组、肾经水肿汤低、中、高剂量组与阳性对照组的 50 只大鼠通过腹膜内注射链脲佐菌素构建大鼠DN模型。记录大鼠一般情况、体重与血糖水平。利用全自动生化分析仪检测各组大鼠的血清肌酐(Cr)与血清尿素氮(BUN)水平。利用高碘酸希夫(PAS)染色检测大鼠肾脏组织的病理变化。蛋白质免疫印迹测定各组大鼠肾脏组织中促血管生成素 1(Ang-1)、激活素样激酶1(ALK1)与转化生长因子-β(TGF-β1)的蛋白水平。结果:正常对照组大鼠的健康状态良好,DN模型组大鼠健康状态较差,出现DN的典型症状。经过肾经水肿汤或 α-硫辛酸治疗后,上述症状均有所减轻。与正常对照组相比,其余各组大鼠的体重均显著下降,血糖水平与血清Cr与BUN水平均显著提高(P<0。05)。与 DN 模型组相比,肾经水肿汤低、中、高剂量组与阳性对照组的体重均显著提高,血糖水平与血清Cr与BUN水平均显著下降,且具有浓度依赖性(P<0。05)。与正常对照组相比,其余各组大鼠的肾组织切片均显示间质增生,肾小球基底膜增厚。与DN模型组相比,肾经水肿汤低、中、高剂量组与阳性对照组的肾组织病变程度均显著减弱,且具有浓度依赖性。与正常对照组相比,DN 模型组大鼠的肾脏组织中Ang-1、TGF-β1 与 ALK1 的蛋白表达水平均显著提高(P<0。01)。与 DN 模型组相比,肾经水肿汤低、中、高剂量组与阳性对照组的肾脏组织中 Ang-1、TGF-β1 与 ALK1 的蛋白表达水平均显著下降,且具有浓度依赖性(P<0。05)。肾经水肿汤高剂量组与阳性对照组无显著性差异(P>0。05)。结论:肾经水肿汤通过抑制TGF-β1/ALK1 信号通路进而降低DN大鼠的血糖水平、肾功能损伤、肾脏血管生成与肾结构变化。
Protective Effects and Regulatory Mechanisms of Shenjingshuizhong Decoction on Diabetic Nephropathy in Rats
Objective:To investigate the protective effect and regulatory mechanism of Shenjing-shuizhong decoction on diabetic nephropathy(DN)rats.Methods:Sixty SPF SD male rats were randomly di-vided into 6 groups(n=10/group):normal control group,DN model group,Shenjingshuizhong decoction low-dose group(1g/kg),Shenjingshuizhong decoction medium-dose group(2g/kg),Shenjingshuizhong decoc-tion high-dose group(4g/kg)and positive control group(α-lipoic acid,60mg/kg).Among them,50 rats in DN model group,Shenjingshuizhong decoction low,medium and high dose groups and positive control group were injected with streptozotocin to construct DN model.The general condition,body weight and blood glucose level of rats were recorded.The levels of serum Creatinine(Cr)and Blood Urea Nitrogen(BUN)were detected by automatic biochemical analyzer.Periodic Acid-Schiff(PAS)staining was used to detect the pathological changes of rat kidney tissue.The protein levels of angiopoietin-1(Ang-1),activin-like kinase 1(ALK1)and transforming growth factor-β(TGF-β1)in kidney tissue were detected by Western blot.Re-sults:Rats in the normal control group were in good health,while those in the DN model group showed poor health with typical symptoms of DN.After treatment with Shenjingshuizhong decoction or α-lipoic acid,the a-bove symptoms were alleviated.Compared with the normal control group,rats of other groups showed that body weight decreased significantly,and the blood glucose level,serum Cr,and BUN levels increased significantly(P<0.05).Compared with the DN model group,the body weight of the low,Shenjingshuizhong decoction medium and high dose groups and the positive control group increased significantly,and the blood glucose lev-el,serum Cr and BUN levels decreased significantly in a concentration-dependent manner(P<0.05).Com-pared with the normal control group,the renal tissue sections of the other groups showed interstitial hyperplasia and thickening of the glomerular basement membrane.Compared with the DN model group,the degree of re-nal tissue lesions in the Shenjingshuizhong decoction low,medium and high dose groups and the positive con-trol group was significantly reduced,and it was concentration-dependent.Compared with the normal control group,the protein expression levels of Ang-1,TGF-β1 and ALK1 in the kidney tissue of the DN model group were significantly increased(P<0.01).Compared with the DN model group,the protein expression levels of Ang-1,TGF-β1 and ALK1 in the kidney tissue of the Shenjingshuizhong decoction low,medium and high dose groups and the positive control group were significantly decreased in a concentration-dependent manner(P<0.05).There was no significant difference between the Shenjingshuizhong decoction high-dose group and the positive control group(P>0.05).Conclusion:Shenjingshuizhong decoction can reduce blood glucose level,renal function injury,renal angiogenesis,and renal structure changes in DN rats by inhibiting TGF-β1/ALK1 signaling pathway.
