摘要

目的:探究癫痫患者脑电图异常和 BDNF、GFAP、miR-7-5p 表达特征,并探究各指标与患者认知功能的关系.方法:本研究以我院2017 年 4 月至 2021 年 4 月期间治疗的 294 例癫痫患者为对象,采用MoCA评分对患者的认知功能进行评价,并据此对患者进行分组:将得分≥26 分的 198 例患者纳入认知正常组,得分<26 分的 96 例患者纳入认知障碍组,患者均接受脑电图检查、血清 BDNF、GFAP、miR-7-5p检测,比较组间脑电图检查异常情况及血清 BDNF、GFAP、miR-7-5p 表达差异,并探究上述指标与认知障碍的关联.结果:对两组患者的MoCA评分进行分析可知认知障碍组患者的Mo-CA各项评分均低于认知正常组(P<0.05).与认知正常组患者相比,认知障碍组患者脑电图无异常占比降低,认知障碍组患者脑电图痫样放电、脑电图慢波发放的发生率高于认知正常组(P<0.05).与认知正常组患者相比较,认知障碍组BDNF、miR-7-5p 降低,GFAP 升高(P<0.05).通过Pearson 相关性分析发现,患者的MoCA评分与血清中的BDNF和miR-7-5p表达呈正相关,与其GFAP 表达呈负相关(P<0.05).经ROC曲线分析,血清 BDNF、miR-7-5p 及 GFAP 表达对癫痫患者认知障碍的发生具有一定诊断价值,其AUC值分别为0.836、0.845、0.957.结论:伴有认知障碍的癫痫患者脑电图异常特征以脑电图痫样放电、脑电图慢波发放为主,血清BDNF、miR-7-5p表达低于认知正常的癫痫患者,GFAP表达高于认知正常的癫痫患者,且血清BDNF、miR-7-5p 及 GFAP 表达对癫痫患者认知障碍的发生具有一定诊断价值.

Abstract

Objective:To explore the relationship between EEG abnormalities,serum levels of Brain-Derived Neurotrophic Factor(BDNF),Glial Fibrillary Acidic Protein(GFAP),and miR-7-5p expression characteristics,and cognitive function in epilepsy patients.Methods:The study included 294 epilepsy pa-tients treated at our hospital from April 2017 to April 2021.Cognitive function was assessed using the Montreal Cognitive Assessment(MoCA)scale.Patients were divided into two groups based on their MoCA scores:198 patients with scores≥26 were included in the cognitively normal group,while 96 patients with scores<26 were included in the cognitive impairment group.All patients underwent EEG examinations.The study com-pared EEG abnormalities and serum levels of BDNF,GFAP,and miR-7-5p between the groups and investi-gated their association with cognitive impairment.Results:Analysis of the MoCA scores showed that patients in the cognitive impairment group had significantly lower scores than those in the cognitively normal group(P<0.05).Compared to the cognitively normal group,patients in the cognitive impairment group had a lower proportion of normal EEGs and higher rates of epileptiform discharges and slow-wave activity(P<0.05).Ser-um levels of BDNF and miR-7-5p were lower,while GFAP levels were higher in the cognitive impairment group(P<0.05).Pearson correlation analysis indicated that cognitive function in epilepsy patients was posi-tively correlated with BDNF and miR-7-5p levels and negatively correlated with GFAP levels(P<0.05).ROC curve analysis demonstrated that BDNF,miR-7-5p,and GFAP levels have diagnostic value for cogni-tive impairment in epilepsy patients,with AUC values of 0.836,0.845,and 0.957,respectively.Conclu-sion:The abnormal characteristics of EEG in patients with cognitive impairment are mainly EEG discharge and slow wave distribution of EEG.The expression of serum BDNF and miR-7-5p is lower than that of patients with normal epilepsy,and the expression of GFAP is higher than that of patients with normal epilepsy,and se-rum BDNF,miR-7-5p and GFAP have certain diagnostic value for the occurrence of cognitive impairment in patients with epilepsy.
