超声血流参数与乳头状甲状腺癌临床病理及超声TI-RADS分级关系研究

目的:分析超声血流参数与乳头状甲状腺癌临床病理及超声TI-RADS分级关系。方法:以2020 年10 月至2023 年11 月于本院确诊的乳头状甲状腺癌患者 103 例为研究对象,所有研究对象均进行超声检查获取血流参数,并进行B超TI-RADS 分级。比较不同病理特征及 TI-RADS 分级患者的血流参数,分析超声血流参数与乳头状甲状腺癌病理及 TI-RADS 分级的相关性。结果:Ⅲ~Ⅳ期患者的搏动指数(PI)、阻力指数(RI)、收缩期峰值流速与舒张期峰值流速比值(S/D)高于Ⅰ~Ⅱ期患者(P<0。05);颈部淋巴结转移患者的PI、RI、S/D显著高于未转移患者(P<0。05);TI-RADS 分级 5~6 级患者的PI、RI、S/D高于4a~4c级(P<0。05)。Spearman 秩相关性分析结果显示,PI、RI、S/D 与乳头状甲状腺癌患者的TNM分期及TI-RADS 分级均呈正相关(rTNM 分期=0。401,0。453,0。462;rTI-RADS分级=0。308,0。376,0。368,P<0。05)。结论:超声血流参数 PI、RI、S/D 与乳头状甲状腺癌患者的肿瘤分期及TI-RADS分级密切相关,早期测定超声血流定量参数可为乳头状甲状腺癌的临床诊断、预测提供一定的参考信息。
Analysis of the Relationship between Ultrasound Blood Flow Parameters and Clinical Pathology and Ultrasound TI-RADS Grading of Papillary Thyroid Cancer
Objective:To analyze the relationship between ultrasound blood flow parameters and clinical pathology and ultrasound TI-RADS grading of papillary thyroid cancer.Methods:A total of 103 patients with papillary thyroid cancer diagnosed in our hospital from October 2020 to November 2023 were enrolled in the study.All subjects underwent ultrasound examination to obtain blood flow parameters and B-ultrasound TI-RADS grading.The blood flow parameters of patients with different pathological features and TI-RADS grades were compared,and the correlation between ultrasound blood flow parameters and papillary thyroid cancer pa-thology and TI-RADS grading was analyzed.Results:The pulsatility index(PI),resistance index(RI),and peak systolic velocity to peak diastolic velocity ratio(S/D)in patients with stages Ⅲ~Ⅳ were higher than those in patients with stages Ⅰ~Ⅱ(P<0.05);The PI,RI,and S/D in patients with cervical lymph node metastasis were significantly higher than those in patients without metastasis(P<0.05);The PI,RI,and S/D in patients with TI-RADS grades 5-6 were higher than those in patients with grades 4a-4c(P<0.05).Spearman rank correlation analysis showed that PI,RI,and S/D were positively correlated with TNM stage and TI-RADS grading in patients with papillary thyroid cancer(rTNM stage=0.401,0.453,0.462;rTI-RADS grading=0.308,0.376,0.368,P<0.05).Conclusion:Ultrasound blood flow parameters PI,RI,and S/D are closely related to tumor staging and TI-RADS grading in patients with papillary thyroid canc-er.Early measurement of ultrasound blood flow quantitative parameters can provide certain reference informa-tion for the clinical diagnosis and prediction of papillary thyroid cancer.
