子宫内膜癌组织中STAT3 ADAMTS19的表达及与病理参数和预后的关系

目的:探讨子宫内膜癌(EC)组织中信号传导及转录激活蛋白3(STAT3)、A解聚素和金属蛋白酶及血小板反应蛋白基序19(ADAMTS19)的表达及与病理参数和预后的关系。方法:选取我院2018年1月至2020年12月接受手术切除的EC患者82例,采用免疫组织化学法检测EC组织及癌旁组织STAT3、ADAMTS19表达。采用Phi相关系数分析EC组织中STAT3、ADAMTS19表达的相关性。根据EC组织STAT3、ADAMTS19表达分为STAT3、ADAMTS19阳性/阴性表达组,采用Kaplan-Meier法绘制STAT3、ADAMTS19阳性/阴性表达EC患者生存曲线,并通过Cox回归分析影响EC患者预后的因素;绘制ROC曲线评价癌组织STAT3、ADAMTS19表达对EC患者死亡的预测价值。结果:与癌旁组织比较,EC组织STAT3阳性表达率升高,ADAMTS19阳性表达率降低(P<0。05)。EC组织中STAT3与ADAMTS19表达呈负相关(φ=-0。443,P<0。001)。STAT3、ADAMTS19表达与EC患者国际妇产科联盟(FIGO)分期、肌层浸润深度、分化程度和淋巴结转移有关(P<0。05)。82例EC患者3年总生存率为69。51%(57/82)。STAT3阳性表达组3年总生存率60。71%低于STAT3阴性表达组88。46%,AD-AMTS19阳性表达组3年总生存率88。89%高于ADAMTS19阴性表达组64。06%(P<0。05)。FIGO 分期Ⅲ期、肌层浸润深度≥ 1/2、低分化、淋巴结转移、STAT3阳性为EC患者死亡的独立危险因素,AD-AMTS 19阳性为独立保护因素(P<0。05)。癌组织STAT3、ADAMTS19表达联合预测EC患者死亡的曲线下面积为0。860,大于癌组织STAT3、ADAMTS19表达单独预测的0。758、0。750(P<0。05)。结论:EC组织STAT3高表达、ADAMTS19低表达,二者表达呈负相关,与FIGO分期、肌层浸润深度、分化程度、淋巴结转移和预后有明显的相关性,且STAT3、ADAMTS19表达联合对EC患者预后有较高预测价值。
Expression of STAT3 and ADAMTS19 in Endometrial Cancer Tissues and Their Relationship with Pathological Parameters and Prognosis
Objective:To investigate the expression of signal transducer and activator of transcription 3(STAT3)and A disintegrin and metalloproteinase with thrombospondin motifs 19(ADAMTS19)in endometri-al carcinoma(EC)tissues and the relationship with pathological parameters and prognosis.Methods:A total of 82 patients with EC who underwent surgical resection in our hospital from January 2018 to December 2020 were selected.Immunohistochemistry was used to detect the expression of STAT3 and ADAMTS19 in EC tis-sues and paracancerous tissues.The correlation between STAT3 and ADAMTS19 expression in EC tissues was analyzed using Phi correlation coefficient.According to the expression of STAT3 and ADAMTS19 in EC tis-sues,the patients were divided into STAT3 and ADAMTS19 positive/negative expression groups.Kaplan-Meier method was used to draw the survival curves of EC patients with STAT3 and ADAMTS19 positive/nega-tive expression,and Cox regression analysis was used to analyze the factors affecting the prognosis of EC pa-tients;ROC curve was used to evaluate the predictive value of STAT3 and ADAMTS19 expression in cancer tissues for the death of EC patients.Results:Compared with paracancerous tissues,the positive expression rate of STAT3 in EC tissues was increased,and the positive expression rate of ADAMTS19 was decreased(P<0.05).The expression of STAT3 and ADAMTS19 in EC tissues was negatively correlated(φ=-0.443,P<0.001).The expression of STAT3 and ADAMTS19 was related to the International Federation of Gynecology and Obstetrics(FIGO)stage,depth of myometrial invasion,degree of differentiation and lymph node metasta-sis of EC patients(P<0.05).The 3-year overall survival rate of the 82 patients with EC was 69.51%(57/82).The 3-year overall survival rate of the STAT3 positive expression group was 60.71%,which was lower than that of the STAT3 negative expression group(88.46%),and the 3-year overall survival rate of the AD-AMTS19 positive expression group was 88.89%,which was higher than that of the ADAMTS19 negative ex-pression group(64.06%)(P<0.05).FIGO stage Ⅲ,myometrial invasion depth ≥ 1/2,low differentiation,lymph node metastasis and STAT3 positive expression were independent risk factors for the death of EC pa-tients,and ADAMTS19 positive expression was an independent protective factor(P<0.05).The area under the curve(AUC)of the joint prediction of STAT3 and ADAMTS19 expression in cancer tissues for the death of EC patients was 0.860,which was greater than that of the separate prediction of STAT3 and ADAMTS19 ex-pression in cancer tissues(0.758,0.750)(P<0.05).Conclusion:STAT3 was highly expressed and AD-AMTS19 was lowly expressed in EC tissues,and their expressions were negatively correlated.They were sig-nificantly related to FIGO stage,depth of myometrial invasion,degree of differentiation,lymph node metasta-sis and prognosis.The joint expression of STAT3 and ADAMTS19 had a high predictive value for the prognosis of EC patients.
