CTRP-3 CTRP12水平与慢性阻塞性肺疾病GOLD分级及预后的相关性研究

目的:分析 C1q 肿瘤坏死因子相关蛋白 3(CTRP-3)，肿瘤坏死因子相关蛋白 12(CTRP12)水平与慢性阻塞性肺疾病(COPD)全球倡议(GOLD)分级及预后的相关性。方法:选取本院2022 年9 月至2024 年1 月收治的 126 例COPD患者作为病例组，选取同时期入院体检且健康的 90 例志愿者为对照组，比较两组临床资料和血清CTRP-3，CTRP12 水平，依据全球倡议(GOLD)分级将患者分为Ⅰ级组(n=9)、Ⅱ级组(n=21)、Ⅲ级组(n=43)和Ⅳ级组(n=53)，对比四组患者CTRP-3，CTRP12水平;记录病例组预后情况，并比较不同预后患者CTRP-3，CTRP12 水平;采用Pearson相关性分析探讨临床指标与血清CTRP-3、CTRP12 水平的相关性。结果:两组年龄、性别、体质量指数、吸烟史及基础疾病比较，差异均无统计学意义(P>0。05);病例组患者WBC 高于对照组(P<0。05)，FEV1、FEV1/FVC 均低于对照组(P<0。05)。病例组 CTRP-3、CTRP12 水平均低于对照组(P<0。05)。不同 GOLD 分级COPD患者组间CTRP-3、CTRP12 水平比较差异有统计学意义(P<0。05);CTRP-3、CTRP12 水平比较均为Ⅰ级组>Ⅱ级组>Ⅲ级组>Ⅳ级组(P<0。05)。预后良好组 CTRP-3、CTRP12 水平均高于预后不良组(P<0。05)。相关性分析显示:COPD患者血清中CTRP-3 水平与吸烟史及GOLD分级均呈负相关(P<0。05)，与FEV1/FVC水平呈正相关(P<0。05);CTRP12 水平与吸烟史、WBC、GOLD 分级均呈负相关(P<0。05)，与FEV1/FVC水平呈正相关(P<0。05)。CTRP-3、CTRP12 水平与COPD病程、住院时间及FEV1 均无相关性(P>0。05)。结论:COPD 患者血清中 CTRP-3、CTRP12 水平低于健康者，且与吸烟史、GOLD分级均成负相关，与FEV1/FVC水平呈正相关。
Correlation between CTRP-3 CTRP-12 Levels and GOLD Grading and Prognosis of Chronic Obstructive Pulmonary Disease
Objective:To analyze the correlation between levels of C1q tumor necrosis factor-related pro-tein 3(CTRP-3)and C1q tumor necrosis factor-related protein 12(CTRP12)and global initiative(GOLD)grading and prognosis of chronic obstructive pulmonary disease(COPD).Methods:A total of 126 patients with COPD in the hospital were selected from September 2022 to January 2024 and included in the case group,and 90 healthy volunteers receiving physical examination were assigned into the control group.The clinical data and serum CTRP-3 and CTRP12 levels were compared between both groups.According to the GOLD grading,COPD patients were classified into grade Ⅰ group(n=9),grade Ⅱ group(n=21),grade Ⅲ group(n=43)and grade Ⅳ group(n=53).The levels of CTRP-3 and CTRP12 among the four groups were compared.The prognosis in the case group was recorded,and the CTRP-3 and CTRP12 levels were compared among patients with different prognosis status.Pearson correlation analysis was adopted to explore the correla-tion between clinical indicators and levels of serum CTRP-3 and CTRP12.Results:The age,gender,body mass index,smoking history and underlying diseases between the two groups manifested no statistical differ-ences(P>0.05).WBC in the case group was higher(P<0.05)while FEV1 and FEV1/FVC were lower than those in the control group(P<0.05).The levels of CTRP-3 and CTRP12 were lower in the case group(P<0.05).There were statistical differences in the levels of CTRP-3 and CTRP12 among COPD patients with different GOLD grades(P<0.05).CTRP-3 and CTRP12 levels were manifested as grade Ⅰ group>grade Ⅱ group>grade Ⅲ group>grade Ⅳ group(P<0.05).Compared with the poor prognosis group,the levels of CTRP-3 and CTRP12 in the good prognosis group were higher(P<0.05).Correlation analysis showed that serum CTRP-3 level in COPD patients was negatively correlated with smoking history and GOLD grading(P<0.05),and was positively associated with FEV1/FVC(P<0.05).CTRP12 level was negatively correlated with smoking history,WBC and GOLD grading(P<0.05),and was positively correlated with FEV1/FVC(P<0.05).The levels of CTRP-3 and CTRP12 were not correlated with the duration of COPD,hospital stay and FEV1(P>0.05).Conclusion:Serum levels of CTRP-3 and CTRP12 in COPD patients are lower com-pared with those of healthy people,and are negatively correlated with smoking history and GOLD grading,and positively correlated with FEV1/FVC level.
