四妙丸联合股四头肌功能锻炼对湿热型膝骨关节炎患者膝关节功能及炎症反应的影响

摘要
目的 观察四妙丸联合股四头肌功能锻炼在湿热型膝骨关节炎(KOA)患者中的应用效果.方法 将 84例湿热型KOA患者按照随机数字表法分为2 组,对照组42 例予股四头肌功能锻炼,治疗组 42 例在对照组治疗基础上予四妙丸治疗.2 组均治疗4 周.比较2 组治疗前后西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分、疼痛视觉模拟评分(VAS)及膝关节液炎症因子[白细胞介素1β(IL-1β)、白细胞介素6(IL-6)]水平,并统计2 组疗效.结果 治疗组总有效率92.86%(39/42),对照组总有效率76.19%(32/42),治疗组临床疗效优于对照组(P<0.05).2 组治疗后WOMAC各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05).2 组治疗后关节液IL-1β、IL-6 水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05).2 组治疗后疼痛VAS均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05).结论 四妙丸联合股四头肌功能锻炼治疗湿热型KOA疗效明显,不仅能缓解疼痛,还能抑制炎症反应,改善膝关节功能.
Abstract
Objective To investigate the effect of Simiao Pills combined with quadriceps functional exercise in patients with knee osteoarthritis(KOA).Methods Eighty-four KOA patients(damp-heat type)were randomly assigned to receive quadriceps functional exercise(the control group,n=42)or Simiao Pills plus quadriceps functional exercise(the treatment group,n=42).Both groups were treated for4 weeks.The aim was to compare the scores of Western Ontario and McMaster Uni-versities Osteoarthritis Index(WOMAC)and the visual analogue scale(VAS),the levels of inflammatory factors(interleukin-1β[IL-1β]and interleukin-6[IL-6])before and after treatment between groups.The curative effects of the two groups were statistically analyzed.Results The overall effective rate of treatment group was significantly higher than that of control group(92.86%[39/42]vs76.19%[32/42],P<0.05).After treatment,the scores of WOMAC and VAS,and the levels of IL-1β and IL-6 in the both groups were significantly lower than those before treatment(P<0.05),and the decreases were more obviously in the treatment group compared to the control group(P<0.05).Conclusion Simiao Pills combined with quadriceps functional exercise has obvious curative effect in the treatment of KOA(damp-heat type),which can not only relieve pain,but also rapidly subside of inflammatory response,with sig-nificant effect in the medium and long term.
