摘要
目的 观察童趣化干预联合中西医结合疼痛管理改善创伤性骨折患儿疼痛及心理的临床疗效.方法 将72 例创伤性骨折患儿按照随机数字表法分为2 组,对照组36 例予常规护理,研究组 36 例予童趣化干预联合中西医结合疼痛管理,2 组均干预7 天.比较2 组干预后疼痛视觉模拟评分(VAS)变化;比较 2 组干预后儿童焦虑性情绪障碍筛查量表(SCARED)评分变化;比较2 组骨折愈合时间、肿胀消失时间;比较2 组干预后炎症因子水平变化;比较2 组患儿依从性.结果 研究组干预后3、7 天疼痛VAS评分均低于对照组同期(P<0.05).研究组SCARED评分(12.61±8.92)分,对照组SCARED评分(28.43±9.27)分,研究组SCARED评分低于对照组(P<0.05).研究组骨折愈合时间、肿胀消失时间均短于对照组(P<0.05).研究组干预后超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)水平均低于对照组(P<0.05).研究组依从性高于对照组(P<0.05).结论 童趣化干预联合中西医结合疼痛管理能有效改善创伤性骨折患儿疼痛程度、心理状况及炎症水平,缩短创伤性骨折患儿骨折愈合时间和肿胀消失时间,提高创伤性骨折患儿依从性.
Abstract
Objective To study the improvement effect of nursing intervention of children's fun mode combined with integrative Western and traditional Chinese medicine model for pain management on pain and psychology in children with traumat-ic fractures.Methods Totally 72 patients with traumatic fractures were randomly assigned to receive either conventional care(n=36,the control group)or nursing intervention of children's fun mode combined with integrative Western and traditional Chi-nese medicine model for pain management(n=36,the study group).A 7-day intervention was performed.The aim was to compare visual analogue scale(VAS)for pain,Screen for Child Anxiety Related Emotional Disorders(SCARED),fracture healing time,disappearing time of swelling,inflammation levels,compliance.Results The VAS scores on days 3 and 7 after intervention in the study group were significantly lower than those of the control group(P<0.05).The SCARED score in the study group was significantly lower than that in the control group([12.61±8.92]points vs[28.43±9.27]points,P<0.05).Significantly shorter fracture healing time and disappearing time of swelling,and significantly lower high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor α(TNF-α)were found in the study group than those of the control group(all P<0.05).The compliance was significantly higher in the study group than in the control group(P<0.05).Conclusion For children with traumatic fractures,nursing intervention of children's fun mode combined with integrative Western and tradition-al Chinese medicine model for pain management can effectively improve the severity of pain,psychological status and inflamma-tion level,shorten the fracture healing time and disappearing time of swelling,and improve the compliance.
