摘要
目的 基于古今医案云平台研究针灸治疗失眠症的选穴规律.方法 检索中国知网建库至2022 年12 月31 日收录的针灸治疗失眠症的中文临床研究文献,提取针灸处方并创建失眠针灸处方数据库,运用古今医案云平台(V2.4.3)的数据挖掘模块功能对数据进行分析.结果 最终纳入文献 431 篇,共提取 974 条针灸处方,涉及腧穴 166个,应用频次前5 位的腧穴为神门(657 次)、百会(576 次)、三阴交(544 次)、内关(432 次)、四神聪(415 次),以足太阳膀胱经腧穴应用频次最多;选穴主要分布在下肢部、头面颈项部;特定穴以交会穴应用最多;关联规则分析得出关联性最强的腧穴组合为三阴交-神门;经社团分析及复杂网络分析可得神门、三阴交、百会、内关、四神聪五个腧穴为主要核心穴组;聚类分析得到12 个聚类群,可分为主穴与配穴两大类.结论 针灸治疗失眠症的选穴以足太阳膀胱经腧穴为主,多运用下肢部、头面颈项部腧穴,临床可以神门、三阴交、百会、内关、四神聪为主穴,结合不同证型配合其他穴位进行施治.
Abstract
Objective To study the acupoint selection rule for acupuncture treatment of insomnia based on the Ancient and Modern Medical Case Cloud Platform.Methods Clinical research literatures of acupuncture for insomnia published by the China National Knowledge Infrastructure(CNKI)from the establishment of the databases to December 31,2022 were retrieved.Acupuncture prescriptions for insomnia were extracted to create a database.The data mining function of the Ancient and Modern Medical Case Cloud Platform(V2.4.3)was used to analyze the data.Results A total of 974 prescriptions were extracted from 431 articles,involving 166 acupoints.The top 5 acupoints were Shenmen(HT7,657 times),Baihui(GV 20,576 times),Sa-nyinjiao(SP 6,544 times),Neiguan(PC 6,432 times),and Sishencong(EX-HN 1,415 times).The most selected meridi-an was the bladder meridian of Foot-Taiyang.The acupoints were mainly distributed in the lower limbs,the head,face and neck.The mostly used specific acupoints were rendezvous acupoints.The correlation rule analysis showed that the most correlated acupoints combination was Sanyinjiao(SP 6)-Shenmen(HT7).Through the community analysis and complex network analy-sis,the main core acupuncture group composed of five acupoints,namely the Shenmen(HT7),Sanyinjiao(SP 6),Baihui(GV 20),Neiguan(PC 6),and Sishencong(EX-HN 1),could be derived.Totally 12 clusters were yielded from the cluster analy-sis,which can be divided into two categories:principal points and adjunct acupuncture points.Conclusion The priority acu-point selection for acupuncture treatment of insomnia are the acupoints along the bladder meridian of Foot-Taiyang.The acu-points in the lower limbs,the head,face and neck are used frequently.Clinically,Shenmen(HT7),Sanyinjiao(SP 6),Baihui(GV 20),Neiguan(PC 6),and Sishencong(EX-HN 1)can be the main acupoints,combined with different syndrome types and supplemented by other acupoints.
