Effect of Yajia Formula combined with Pingying Powder for thyroid peroxidase antibody,transforming growth factor-beta1 and thyroglobulin antibody in patients with subacute thyroiditis

目的 观察亚甲方联合平瘿散对亚急性甲状腺炎患者的治疗效果,及对患者血清过氧化物酶抗体(TPO-Ab)、转化生长因子β1(TGF-β1)和甲状腺球蛋白抗体(TgAb)的影响.方法 将102例SAT患者按照随机数字表法分为2组,对照组51例予常规西药治疗,治疗组51例予亚甲方口服联合平瘿外敷治疗,治疗4周后评估2组临床疗效.比较2组治疗前后中医症状评分及血清TPO-Ab、TGF-β1、TgAb、促甲状腺激素(TSH)、游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)水平和131I甲状腺摄取率变化.结果 治疗组总有效率98.04％(50/51),对照组总有效率98.04％(50/51),2组临床疗效比较差异无统计学意义(P＞0.05).与本组治疗前相比,治疗后2组甲状腺痛、发热、咽痛、心悸、乏力、烦躁、消瘦评分均下降(P＜0.05),且治疗后治疗组甲状腺痛、咽痛、心悸、乏力、烦躁、消瘦评分均低于对照组(P＜0.05).与本组治疗前相比,治疗后2组TPO-Ab、TGF-β1、TgAb水平均下降(P＜0.05),且治疗后治疗组TGF-β1水平低于对照组(P＜0.05).与本组治疗前相比,治疗后2组血清FT3、FT4水平均显著降低(P＜0.05),TSH水平显著升高(P＜0.05),且治疗后治疗组血清FT3、FT4水平低于对照组(P＜0.05).与本组治疗前相比,治疗后2组2 h、6 h、24 h 131I甲状腺摄取率均升高(P＜0.05),且治疗后治疗组2 h、6 h、24 h 131I摄入率均高于对照组同期(P＜0.05).结论 亚甲方联合平瘿散治疗SAT,能改善患者甲状腺功能和症状,降低TGF-β1水平.
Objective To analyze the therapeutic effect of Yajia Formula combined with Pingying Powder for patients with subacute thyroiditis(SAT),and its influence on thyroid peroxidase antibody(TPO-Ab),transforming growth factor-beta1(TGF-β1)and thyroglobulin antibody(TgAb).Methods Totally 102 SAT patients were randomised 1∶1 to receive routine Western medicine(RWM)(control group)or oral Yajia Formula+external application with Pingying Powder(treatment group).Treated for 4 weeks,the aim was to compare traditional Chinese medicine(TCM)syndrome score,serum TPO-Ab,TGF-β1,TgAb,thyroid stimulating hormone(TSH),free thyroxine(FT4),free triiodothyronine(FT3),thyroid 131I uptake measurements.The curative effect was assessed.Results The overall effective rate in the treatment group was comparable to the control group(98.04％[50/51]vs 98.04％[50/51],P＞0.05).After treatment,TCM symptom(thyroid pain,fever,sore throat,palpitations,fatigue,irritability,weight loss)scores in groups were significantly decreased(P＜0.05),TCM symptom(thyroid pain,sore throat,palpitations,fatigue,irritability,weight loss)scores decreased notably in the treatment group compared with the control group(P＜0.05).TPO-Ab,TGF-β1,TgAb levels in groups were significantly decreased(P＜0.05),TGF-β,was significantly lower in the treatment group than in the control group(P＜0.05).After treatment,significantly decreased FT3 and FT4,and increased TSH were found in the two groups(all P＜0.05).FT3 and FT4 were significantly lower in the treatment group than in the control group(P＜0.05).After treatment,2 h,6 h,24 h thyroid 131I intake rate in groups increased significantly(P＜0.05),the increase in the treatment group was more common than the control group(P＜0.05).Conclusion Yajia Formula combined with Pingying Powder for SAT patients can improve patients'thyroid function and symptoms,and reduce TGF-β1 levels.
