经皮穴位电刺激疗法联合康复干预治疗脑卒中偏瘫疗效研究

摘要
目的 观察经皮穴位电刺激疗法联合康复干预治疗脑卒中偏瘫的临床效果.方法 将我院120例脑卒中偏瘫患者随机分为治疗组与对照组各60例.对照组用采康复干预治疗,治疗组采用经皮穴位电刺激疗法联合康复干预治疗.比较两组临床总有效率、中医主症积分、即刻体感诱发电位(SEP)N20波幅,评估比较上肢、下肢Fugl-Meyer量表(FMA)、Holden功能性步行量表(Holden)评分及伸肌、屈肌峰力矩.结果 治疗后,治疗组临床总有效率显著高于对照组(P＜0.05);两组中医主症积分均显著低于治疗前,治疗组中医主症积分均显著低于对照组(P＜0.05);两组SEPN20波幅显著高于治疗前,治疗组SEPN2.波幅显著高于对照组(P＜0.05);两组上肢、下肢FMA、Holden评分均显著高于治疗前,治疗组上肢、下肢FMA、Holden评分均显著高于对照组(P＜0.05);两组伸肌、屈肌峰力矩均显著高于治疗前,治疗组伸肌、屈肌峰力矩均显著高于对照组(P＜0.05).结论 经皮穴位电刺激疗法联合康复干预显著提高治疗脑卒中偏瘫患者的疗效,降低中医主症积分,恢复中枢神经功能,改善上肢、下肢运动功能及步行功能,提高肌力.
Abstract
Objective To observe the clinical effect of percutaneous acupoint electrical stimulation therapy combined with rehabilitation intervention on stroke hemiplegia.Methods A total of 120 stroke hemiplegic patients were randomly divided into treatment group and control group,with 60 cases in each group.Control group received rehabilitation intervention treatment,while treatment group received percutaneous acupoint electrical stimulation therapy on the basis of control group.Compared the total clinical effective rates between the two groups,recorded and compared the TCM main symptom scores of the two groups,tested and compared the amplitude of immediate somatosensory evoked potential(SEP)N20,evaluated and compared the Fugl Meyer Scale(FMA)and Holden Functional Walking Scale(Holden)scores of the upper and lower limbs,and detected and compared the peak torque of the extensor and flexor muscles of the two groups.Results After treatment,the total clinical effective rate of treatment group was significantly higher than control group(P＜0.05);the TCM main symptom scores of both groups significantly decreased than those before treatment,and were significantly lower in treatment group than control group(P＜0.05);the amplitude of SEPN20 waves significantly increased in both groups than those before treatment,and were significantly higher in treatment group than control group(P＜0.05);the FMA and Holden scores of the upper and lower limbs in both groups were significantly higher than before treatment,and were significantly higher in treatment group than control group(P＜0.05);the peak torque of the extensor and flexor muscles in both groups significantly increased than before treatment,and was significantly higher in treatment group than control group(P＜0.05).Conclusion The combination of percutaneous acupoint electrical stimulation therapy and rehabilitation intervention significantly improves the therapeutic effect of treating stroke hemiplegia patients,reduces the main TCM symptom scores,restores central nervous system function,improves upper and lower limb motor function and walking function,and enhances muscle strength.
