摘要
目的 观察丹参注射液联合坎地沙坦酯片治疗老年慢性心力衰竭患者临床疗效及其对心功能指标、血液流变学和血清指标的影响.方法 将我院收治的84例老年慢性心力衰竭患者随机分为西医组与联合组各42例.西医组给予坎地沙坦酯片治疗,联合组在西医组的治疗基础上给予丹参注射液.比较两组患者的治疗总有效率,心功能指标左心室收缩末期体积(LVESV)、左心室舒张末期体积(LVEDV)、左心室射血分数(LVEF),血液流变学指标红细胞压积(HCT)、D-二聚体(D-D)、纤维蛋白原(FIB),血清指标N-末端脑钠肽前体(NT-proBNP)、同型半胱氨酸(Hcy)以及不良反应情况.结果 治疗后,联合组治疗总有效率高于西医组(P＜0.05);两组LVESV、LVEDV、HCT、D-D、FIB、NT-proBNP、Hcy水平均下降,且联合组上述指标均低于西医组(P＜0.05);两组LVEF水平均上升,且联合组高于西医组(P＜0.05);联合组不良反应率与西医组比较无统计学意义(P＞0.05).结论 丹参注射液联合坎地沙坦酯片治疗老年慢性心力衰竭患者疗效较好,可保护患者心功能,改善血液流变学指标,降低NT-proBNP、Hcy水平且安全性良好.
Abstract
Objective To observe the curative effect of Danshen Injection combined with candesartan cilexetil tablet and its influences on cardiac function indexes,hemorheology,serum indexes in the elderly with chronic heart failure(CHF).Methods A total of 84 elderly patients with CHF were randomly divided into Western medicine group and Chinese-Western combination group,with 42 cases each.Western medicine group received candesartan cilexetil tablets,while Chinese-western combination group received Danshen Injection on the basis of western medicine group.The total response rate of treatment,cardiac function indexes[left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),left ventricular ejection fraction(LVEF)],hemorheology[hematokrit(HCT),D-dimer(D-D),fibrinogen(FIB)],serum indexes[N-terminal pro brain natriuretic peptide(NT-proBNP),homocysteine(Hcy)]and adverse reactions in the two groups were compared.Results After treatment,total response rate of treatment was higher in Chinese-Western combination group than Western medicine group(P＜0.05);Hcy,LVESV,LVEDV,HCT,D-D,FIB,and NT-proBNP were decreased in both groups,which were lower in Chinese-Western combination group(P＜0.05),while LVEF was increased,which was higher in Chinese-Western combination group(P＜0.05).The difference in the incidence of adverse reactions between the two groups was not statistically significant(P＞0.05).Conclusion Danshen Injection combined with candesartan cilexetil tablet has good curative effect in elderly patients with CHF,which can protect cardiac function,improve hemorheology indexes,reduce NT-proBNP and Hcy levels with good safety.
