摘要

巴元明教授根据临床表现将失眠分为入睡困难、夜间易醒、眠浅多梦、醒后不解乏、早醒五种状态,根据黄元御"一气周流"理论将失眠核心病机归纳为中土失运,木火金水四轮运转失常,治疗失眠重在恢复中土之运,及调和全身之阴阳、气机,气血流行,则神志魂魄得以充养潜藏,采用培土疏木、滋水涵木、填补肾精、宣降肺金等法,以五行之生克制化复四轮之职.临床失眠五态往往杂糅,运用"一气周流"理论可将病机化繁为简,具有一定的指导意义.

Abstract

Pro.BA Yuan-ming classified insomnia into five states based on clinical manifestations:difficulty in falling asleep,easy to wake up at night,shallow and dreamy sleep,no fatigue after waking up,and early awakening.According to HUANG Yuan-yu's"Yiqi circulation"theory,the core pathogenesis of insomnia is summarized as loss of energy in the middle earth,abnormal operation of the four wheels of wood,fire,gold,and water.The key to treating insomnia is to restore the energy of the middle earth,and to regulate the yin,yang,and qi mechanisms of the whole body.When qi and blood flow,the mind and soul can be replenished and hidden,by using methods such as cultivating soil and dispersing wood,nourishing water and containing wood,filling kidney essence,and promoting the descending of lung gold,the five elements are used to suppress the function of transforming and restoring the four rounds.Several states of clinical insomnia are often mixed together,and the use of the"Yiqi circulation"theory can simplify the pathogenesis,which has certain guiding significance for the clinical treatment of insomnia.
