妊娠期糖尿病对体外受精-胚胎移植双胎妊娠结局的影响

摘要
目的 探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)对体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)双胎妊娠结局的影响.方法 收集IVF-ET术后合并GDM的孕妇122例,分为双胎妊娠组68例,单胎妊娠组54例;自然双胎妊娠合并GDM组患者12例为对照,比较各组间剖宫产、缺铁性贫血、产后出血、胎膜早破、前置胎盘、胎膜早破、脐带绕颈、胎儿畸形发生率.结果 IVF-ET术后合并GDM双胎组剖宫产率、缺铁性贫血发生率高于IVF-ET术后合并GDM单胎组(P<0.05),两组比较产后出血、胎膜早破、前置胎盘、脐带绕颈、胎儿畸形发生率的差异均无统计学意义(P>0.05);IVF-ET术后双胎合并GDM组胎膜早破发生率低于自然妊娠双胎合并GDM组(P<0.05),两组比较产后出血、缺铁性贫血、前置胎盘、脐带绕颈、胎儿畸形发生率及剖宫产率无显著性差异(P>0.05).结论 妊娠期糖尿病可增加母婴不良妊娠结局的风险,IVF-ET术后双胎合并妊娠期糖尿病增加了剖宫产的风险及贫血的发生率.
Abstract
Objective To investigate the effect of gestational diabetes mellitus (GDM) on twin pregnancy outcome after in vitro fertilization and embryo transfer (IVF-ET).Methods One hundred and twenty-two patients with GDM after IVF-ET were collected and divided into twin pregnancy with GDM after IVF-ET group (n =68), single pregnancy with GDM after IVF-ET group (n =54);and natural twin pregnancy with GDM group (n =12) were as control.The incidence rate of postpartum hemorrhage, iron deficiency anemia, placenta previa, premature rupture, umbilical cord around the neck, fetal malformation and cesarean section in the three groups were compared.Results Incidence rate of cesarean section, iron deficiency anemia in twin pregnancy with GDM after IVF-ET group were higher than those in single pregnancy with GDM after IVF-ET group (P < 0.05), there was no significant difference inpostpartum hemorrhage, placenta previa, premature rupture, umbilical cord around the neck, and fetal malformation between the two groups (P > 0.05);Incidence rate of premature rupture in twin pregnancy with GDM after IVF-ET group was lower than that in natural twin pregnancy with GDM group (P < 0.05), there was no significant difference in postpartum hemorrhage, placenta previa, umbilical cord around the neck, fetal malformation, and cesarean section between the two groups (P > 0.05).Conclusion Pregnancy with gestational diabetes mellitus (GDM) may increase the risk of adverse pregnancy outcomes, and twin pregnancy with GDM after IVF-ET increases the incidence of cesarean section and iron deficiency anemia.
