目的 探讨髁突骨折采用颌后入路手术治疗的临床效果.方法 选取2018年5月~2019年6月在我院治疗的髁突骨折患者84例,采用信封法将患者分为观察组(n=42)和(n=42),观察组给予颌后入路手术治疗,对照组采用耳前入路治疗,观察并记录两组患者的手术时间和术中出血量等,采用华盛顿大学生存质量问卷(University of Washington Quality of Life,UW-QOL)评价患者生存质量.结果 观察组和对照组手术时间、出血量和术后引流量比较差异无统计学意义(P>0.05);观察组术后6个月开口度为(33.41±6.10)mm,明显高于对照组(P<0.05);观察组术后6个月下颌前伸、后退运动幅度分别为(8.50±1.61)mm和(0.68±0.19)mm,明显高于对照组(P<0.05);观察组术后6个月UW-QOL问卷的美容效果、情绪和疼痛评分分别为(87.10±9.82)分、(82.30±8.54)分和(81.01±9.06)分,高于对照组(P<0.05);观察组术后并发症率为4.76%,显著低于对照组(P<0.05).结论 髁突骨折采用颌后入路手术治疗有较好的疗效,值得临床应用.
Analysis of therapeutic effect of the posterior mandibular approach in treat-ment of condylar fracture
Objective To investigate the clinical effect of posterior mandibular approach in treatment of condylar fracture.Methods Eighty four patients with condylar fractures in our hospital from May 2018 to June 2019 was collected.The patients were divided into two groups of 42 each using the envelope method.The observation group received treatment through poste-rior mandibular approach,while the control group was treated using anterior ear approach.Op-eration time,bleeding volume,and other factors were observed and compared.The quality of life of the patients was evaluated using the University of Washington Quality of Life question-naire(UW-QOL).Results There were no significant differences between the observation and control groups in terms of operation time,bleeding volume,and postoperative drainage volume(P>0.05).However,six months after surgery,the observation group showed a significantly larger mouth opening(33.41 mm±6.10 mm),and greater forward and backward jaw move-ment(8.50 mm±1.61 mm and 0.68 mm±0.19 mm,respectively)compared with the control group(P<0.05).Aesthetics,emotional well-being,and pain management on the UW-QOL questionnaire of the observation group were(87.10±9.82),(82.30±8.54),(81.01±9.06),respectivlely,higher than that of the control group(P<0.05).The postoperative complication rate of the observation group was 4.76%,which was significantly lower than that of the control group(P<0.05).Conclusion The treatment of condylar fracture by posterior mandibular approach has good curative effect and is worthy of clinical use.
