目的 观察深层肌肉刺激与有氧训练结合方案对膝关节韧带损伤患者关节及韧带功能康复的影响.方法 选取四川省骨科医院于2020年2月~2023年4月收治的膝关节韧带损伤患者126例,将上述患者采用随机数字表法分为2组,每组63例.所有患者均已接受韧带重建手术,对照组给予有氧训练干预,观察组给予深层肌肉刺激与有氧训练结合方案干预.比较两组韧带功能指标、膝关节活动度、时空参数、简版恐动症评估量表(Tampa Scale of Kinesiophobia,TSK-11)评分、Lysholm膝功能评分、耗时及平均轨迹误差(average trajectory error,ATE)、日常生活能力(activity of daily living,ADL)评分的差异.结果 两组治疗前韧带功能指标比较,差异无统计学意义(P>0.05).两组治疗后肢体对称性测定值、屈膝峰力矩比、伸膝峰力矩比均升高,观察组韧带功能指标较对照组更高(P<0.05).两组治疗前膝关节活动度比较,差异无统计学意义(P>0.05).两组治疗后膝关节屈伸、外旋活动度均升高,观察组膝关节活动度较对照组更高(P<0.05).两组治疗前时空参数比较,差异无统计学意义(P>0.05).两组治疗后步速、患侧步长均升高,观察组步速、患侧步长较对照组更高(P<0.05);两组治疗后患侧触地时长、患侧摆动相对比均降低,观察组患侧触地时长、患侧摆动相对比较对照组更低(P<0.05).两组治疗前ATE及相关评分比较,差异无统计学意义(P>0.05).两组治疗后Lysholm膝功能评分、ADL.评分均升高,观察组Lysholm膝功能评分、ADL评分较对照组更高(P<0.05);两组治疗后ATE、TSK-11评分均降低,观察组ATE、TSK-11评分较对照组更低(P<0.05).结论 深层肌肉刺激与有氧训练结合方案可促进膝关节韧带损伤患者韧带功能及膝关节功能的恢复,提高步速,增大步幅,降低患者恐惧心理,提高日常生活能力.
Effect of combination of deep muscle stimulation and aerobic training on re-habilitation of joint and ligament function in patients with knee ligament in-jury
Objective To observe the effect of deep muscle stimulation combined with aerobic training on the rehabilitation of joint and ligament function in patients with knee ligament injury.Methods A total of 126 patients with knee ligament injury admitted to Sichuan Province Or-thopedic Hospital from February 2020 to April 2023 were selected and divided into two groups by random number table method,with each group of 63 patients.Both groups were given liga-ment reconstruction surgery,the control group was given aerobic training intervention,and the observation group was given deep muscle stimulation combined with aerobic training interven-tion.Ligament function indexes,rang of motion of knee joint,temporal and spatial parameters,Tampa Scale of Kinesiophobia(TSK-11)score,Lysholm knee function score,time consumption and average trajectory error(ATE),and activity of daily living(ADL)scores were compared between the two groups.Results There was no statistical significance in anterior ligament function between the two groups(P>0.05).After treatment,the measurement values of limb symmetry,the ratio of flexion peak moment and the ratio of knee extension peak moment were increased in both groups,and the index of ligament function in the observation group was high-er than that in the control group(P<0.05).There was no significant difference in range of motion of knee joint between the two groups before treatment(P>0.05).After treatment,knee flexion and extension and external rotation of the two groups were increased,and the rang of motion of knee joint of the observation group was higher than that of the control group(P<0.05).There was no statistical significance in temporal and spatial parameters between the two groups before treatment(P>0.05).After treatment,the step speed and the step length of the affected side of the two groups were increased,and the step speed and the step length of the af-fected side of the observation group were higher than that of the control group(P<0.05).Af-ter treatment,the relative ratio of affected side touch time and affected side swing were de-creased in both groups,and those in the observation group were lower than those in the control group(P<0.05).There was no statistical significance in ATE and related scores between the two groups before treatment(P>0.05).Lysholm knee function score and ADL score were in-creased in both groups after treatment,and Lysholm knee function score and ADL score in ob-servation group were higher than those in control group(P<0.05).The ATE and TSK-11 scores of both groups were decreased after treatment,and the ATE and TSK-11 scores of the observation group were lower than those of the control group(P<0.05).Conclusion The combination of deep muscle stimulation and aerobic training can promote the recovery of liga-ment function and knee function in patients with knee ligament injury,improve walking speed,increase stride length,reduce the fear of patients,and improve the ability of daily life.
