Medical and elderly care integration services demand of elderly patients with hypertension and diabetes mellitus in different health dimensions
Aim:To investigate the relationship between health-related quality of life and the demand for integrated medical and elderly care services in elderly patients with hypertension and diabetes mellitus at home in community.Meth-ods:A total of 1 443 elderly people(≥60 years)living in the community in Guangzhou,Suzhou,Jinan,Qingdao and other regions were selected from April to May 2023 using the multi-stage stratified cluster random sampling method,and 254 pa-tients with hypertension and diabetes mellitus were selected as the research subjects.The European five-dimensional health EQ-5D scale was used to investigate the health-related quality of life,and the differences in the demand for integrated medi-cal and elderly care services in the patients in different health dimensions were analyzed.Results:The EQ-5D health utility value of the patients was 0.674.Among the 5 dimensions of health-related quality of life,58.3%of the 254 elderly patients had difficulties in mobility,56.3%had difficulties in daily living ability,53.5%had pain or discomfort,49.2%had diffi-culty in self-care ability,and 27.6%had anxiety or depression.The demand of the elderly patients with mobility difficulties in medical rehabilitation,medical nursing,first aid,clinical treatment,rehabilitation training,door-to-door service,meal as-sistance,toilet assistance,bathing assistance,walking assistance,emergency assistance,and psychological care was higher than those of patients without mobility difficulties(P<0.05).The demand of the elderly patients with difficulties in self-care ability was higher in medical rehabilitation,medical nursing,first aid,rehabilitation training,meal assistance,toilet as-sistance,bathing assistance,walking assistance,emergency assistance,and psychological care than those of patients without(P<0.05).The demand of the elderly patients with difficulties in daily activities was higher in medical rehabilitation,med-ical nursing,first aid,rehabilitation training,meal assistance,toilet assistance,bathing assistance,walking assistance,emer-gency assistance,psychological care,and leisure and entertainment than those of patients without(P<0.05).The demand of the elderly patients with pain or discomfort was higher in door-to-door service,toilet assistance,bathing assistance,walking assistance,and emergency assistance than those without(P<0.05).Conclusion:The health-related quality of life of the elderly patients with hypertension and diabetes mellitus is low;the demand of patients with different health dimensions for home-based medical and elderly care services in community is different.
