This paper introduces an accident of pipeline leakage and fire caused by the replacement of circulating pump.Through on-site investigation,investigation and related reinspection,it is found that the direct cause of the accident is two operators'illegal operation.In the state of not stopping the oil,the replacement gasket with pressure and the inlet valve is not closed,which causes the organic heat carrier to erupt from the replacement of circulating pump and flash fire.The indirect reasons are that the main responsibility for safety production of the enterprise is not in place,the on-site personnel management is not standardized,and the emergency response measures are improper.
