目的 分析补中益气汤联合股骨近端防旋髓内钉(proximal femoral nail antirotation,PFNA)对股骨粗隆间骨折的治疗效果.方法 便利选取2019年6月—2022年6月莆田市中医医院诊治的股骨粗隆间骨折患者102例为研究对象,基于入院先后顺序分为对照组与研究组,各51例,两组患者均接受PFNA手术治疗,分别在术后接受碳酸钙与补中益气汤治疗.对比两组患者髋关节功能恢复优良率、临床指标、血流动力学与并发症发生率.结果 研究组髋关节功能恢复优良率为94.12%,高于对照组的78.43%,差异有统计学意义(χ2= 5.299,P<0.05).治疗后,与对照组比较,研究组骨密度有提高,疼痛评分有降低,步幅与步速有显著提高,差异有统计学意义(P均<0.05).治疗后研究组全血低切黏度、全血高切黏度、血浆黏度表达水平显著低于对照组,差异有统计学意义(P均<0.05).研究组并发症发生率低于对照组,差异有统计学意义(P<0.05).结论 对股骨粗隆间骨折患者开展PFNA手术后加强补中益气汤进行治疗,有助于促进患者髋关节功能恢复,疗效确切,且安全性高.
Study on the Efficacy and Safety of Buzhong Yiqi Decoction Combined with PFNA on Intertrochanteric Fractures of Femur
Objective To analyze the therapeutic effect of Buzhong Yiqi decoction combined with proximal femoral nail antirotation(PFNA)on intertrochanteric fracture of femur.Methods A total of 102 patients with intertrochanteric fracture of femur treated and treated in Putian Hospital of Traditional Chinese Medicine from June 2019 to June 2022 were conveniently selected as the study objects.Based on the order of admission,they were divided into control group and study group,with 51 cases in each group.Both groups received PFNA surgery,and received calcium carbonate and Buzhong Yiqi decoction after surgery,respectively.The excellent and good rate of hip function recovery,clinical indexes,hemodynamics and complication rate were compared between the two groups.Results The rate of hip function recovery in the study group was 94.12%,higher than 78.43%in the control group,and the difference was statistically significant(χ2=5.299,P<0.05).After treatment,compared with the control group,the bone density of the study group was increased,the pain score was decreased,and the stride length and pace were significantly increased,the differ-ences were statistically significant(all P<0.05).After treatment,the expression levels of whole blood low tangential viscosity,whole blood high tangential viscosity and plasma viscosity in the study group were significantly lower than those in the control group,and the differences were statistically significant(all P<0.05).The complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclu-sion Strengthening the treatment of Buzhong Yiqi decoction after PFNA surgery for patients with intertrochanteric frac-ture of femur can help to promote the recovery of hip function of patients,and the therapeutic efficacy is accurate and safety is high.
