目的 探讨以健康行为改变整合理论(Integrated Theory of Health Behaviour Change,ITHBC)为指导的群组式饮食及运动指导对初产妇母婴结局的影响.方法 方便选取2022年1-12月重庆市沙坪坝区妇幼保健院收治的94例初产妇为研究对象,采用随机数表法分为对照组和观察组,各47例.对照组行常规孕期保健服务,观察组采用以ITHBC为指导的群组式饮食及运动指导,比较两组母婴结局、自我管理能力、饮食及运动管理情况、一般自我效能感量表(General Self-Efficacy Scale,GSES)评分.结果 观察组不良母婴结局的发生率为2.13%,低于对照组的17.02%,差异有统计学意义(χ2=6.021,P=0.014).观察组自我管理能力评分、饮食、运动、体质量监测的依从率及体质量控制理想率、GSES评分高于对照组,差异有统计学意义(P均<0.05).结论 在初产妇孕期指导中,以ITHBC为指导的群组式饮食及运动指导能够改善初产妇的母婴结局及饮食、运动管理状况,降低早产、巨大儿等风险,且在提高初产妇自我管理能力、提升自我效能水平等方面有重要作用.
Effects of ITHBC-guided Group Diet and Exercise Guidance on Maternal and Infant Outcomes in First-time Parturients
Objective To explore the effect of group diet and exercise guidance guided by integrated theory of health behaviour change(ITHBC)on maternal and infant outcomes of first-time parturients.Methods A total of 94 cases of primipara treated in Shapingba Maternal and Child Health Hospital of Chongqing City from January to December 2022 were conveniently selected as the study objects,and were divided into control group and observation group by random number table method,with 47 cases in each group.The control group received routine health care during preg-nancy,while the observation group received group diet and exercise guidance guided by ITHBC.The outcome,self-management ability,diet and exercise management,and General Self-Efficacy Scale(GSES)score of the two groups were compared.Results The incidence of adverse maternal and infant outcomes in the observation group was 2.13%,lower than that in the control group(17.02%),and the difference was statistically significant(χ2=6.021,P=0.014).The self-management ability score,the compliance rate of diet,exercise and weight monitoring,the ideal rate of body qual-ity control and the GSES score of the observation group were higher than those of the control group,and the differ-ences were statistically significant(all P<0.05).Conclusion In the guidance of first-time parturients during pregnancy,group diet and exercise guidance guided by ITHBC can improve maternal and infant outcomes,diet and exercise man-agement of first-time parturients,reduce the risk of premature delivery and macrogyne,and play an important role in improving the self-management ability of first-time parturients and enhancing the level of self-efficacy.
