Effects of Hospital-community-family Integrated Nursing on Self-management Efficacy and Blood Glucose Control in Patients with Type 2 Diabetes
Objective To study the effects of hospital-community-family integrated nursing on self-management effi-cacy and blood glucose control in patients with type 2 diabetes.Methods A total of 150 patients with type 2 diabetes admitted to the Second Hospital of Sanming City from November 2019 to November 2021 were simplely randomly se-lected as the study objects and were divided into two groups according to different nursing methods.The control group(n=75)received routine nursing,and the observation group(n=75)received hospital-community-family integrated nursing.The self-management ability,blood glucose control level,psychological state and nursing satisfaction were compared between the two groups.Results The self-management efficacy of the observation group was higher than that of the control group 1 month and 3 months after nursing,and the differences were statistically significant(both P<0.05).Before nursing intervention,there was no statistical significance difference in fasting blood glucose and HBA1c between the two groups(both P>0.05).After 3 months of nursing,the fasting blood glucose and HBA1c indexes of the observation group were lower than those of the control group,and the differences were statistically significant(both P<0.05).After nursing,the Self-Rating Anxiety Scale(SAS)score and Self-Rating Depression Scale(SDS)score of both groups were decreased,and the observation group was lower than the control group,and the differences were statisti-cally significant(all P<0.05).The total satisfaction of the observation group was 98.67%,higher than 89.33%of the control group,and the difference was statistically significant(χ2=4.255,P<0.05).Conclusion The use of"hospital-community-family"integrated nursing model for nursing intervention in type 2 diabetes patients can effectively im-prove the self-management ability and blood glucose level of patients,alleviate their bad emotions,and improve nurs-ing satisfaction.
