Effect of Direct Anterior Approach Hip Replacement in the Treatment of Femoral Neck Fracture in Elderly Patients and Its Influence on Hip Func-tion after Operation
Objective To analyze the effect of direct anterior approach hip replacement on hip function in elderly pa-tients with femoral neck and femoral fracture.Methods A total of 70 elderly patients with femoral neck fracture admit-ted to Zhangping City Hospital from January 2019 to July 2023 were randomly selected as study objects and divided into two groups by random number table method 35 cases in each group.Total hip arthroplasty was performed in both groups.Direct anterior approach was taken in the observation group,while posterolateral approach was taken in the control group.Perioperative indexes,inflammatory indexes,hip function and complications within 3 months after op-eration were compared between the two groups.Results The perioperative indexes in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The levels of postoperative in-flammation indexes in the observation group was lower than that in the control group,and the difference was statisti-cally significant(P<0.05).After operation 1,3 menths,the hip function scores of the observation group were(81.73±5.73)points and(92.56±4.19)points,which were higher than those of the control group(75.68±5.92)points and(86.89±4.47)points,and the differences were statistically significant(t=4.344,5.475,both P<0.05).There was no sig-nificant difference in the total incidence of complications 3 months after operation between the two groups(P>0.05).Conclusion Direct anterior approach for hip replacement can reduce intraoperative blood loss in elderly patients with femoral neck fracture,shorten operation duration,incision length,postoperative bed time and hospital stay,reduce in-flammation and improve hip joint function,with high safety.
