目的 探讨股骨近端抗旋髓内钉(Proximal Femoral Nail Antirotation,PFNA)内固定术治疗股骨粗隆间骨折的疗效以及对患者的手术时间、出血量、术后疼痛和髋关节活动性的影响.方法 简单随机选取2021年6月—2023年5月聊城市冠县人民医院诊治的60例股骨粗隆间骨折患者作为研究对象,采用随机数表法将患者划分为研究组(n=30)和对照组(n=30).对照组采用行动力髋螺钉(Dynamic Hip Screw,DHS)内固定术进行治疗,研究组采用PFNA内固定术治疗.比较两组的手术相关指标、Harris髓关节功能评分优良率、疼痛评分及并发症发生率.结果 研究组手术时间、术中出血量、切口长度、下床活动时长、住院时间以及骨折愈合时间均短于对照组,差异有统计学意义(P均<0.05).研究组的Harris髓关节功能评分优良率(96.67%)高于对照组(73.33%),差异有统计学意义(χ2=7.862,P<0.05).术后,研究组的疼痛评分低于对照组,差异有统计学意义(P<0.05);研究组的并发症发生率低于对照组,差异有统计学意义(P<0.05).结论 在治疗股骨粗隆间骨折的过程中,PFNA的内部固定手术展现出卓越的效果,能有效缩短手术所需的时间,减少出血量,改善髋关节活动度以及术后疼痛情况.
Effect of PFNA Internal Fixation on Operative Time,Blood Loss,Postop-erative Pain and Hip Motion in Patients with Intertrochanteric Fracture of Femur
Objective To investigate the efficacy of Proximal Femoral Nail Antirotation(PFNA)internal fixation for in-tertrochanteric fractures and the effects on operative time,blood loss,postoperative pain and hip mobility.Methods A total of 60 patients with intertrochanteric fracture of femur treated in Guanxian People's Hospital of Liaocheng City from June 2021 to May 2023 were randomly selected as the study objects.The patients were divided into study group(n=30)and control group(n=30)by random number table method.The control group was treated with Dynamic Hip Screw(DHS),and the study group was treated with PFNA internal fixation.The surgery related indexes,excellent rate of Harris pulp joint function score,pain score and complication rate were compared between the two groups.Results The operative time,intraaoperative blood loss,the length of incision,the time to get out of bed,hospital stays and the time for fracture healing in the study group were all shorter than those in the control group,and the differences were statistically significant(all P<0.05).The excellent rate of Harris pulp joint function score of the study group was higher(96.67%)than that of the control group(73.33%),and the difference was statistically significant(χ2=7.862,P<0.05).After operation,the pain score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In the treatment of intertrochanteric fractures of the femur,internal fixation of PFNA has shown remarkable results,shortening the time required for surgery,reducing blood loss,and improving hip motion and postoperative pain.
