Effect of Laparoscopic Surgery at Different Times on the Curative Effect and Complications of Acute Pancreatitis Caused by Gallstones
Objective To analyze the effect and complications of laparoscopic surgery in patients with acute pancreati-tis caused by gallstones at different times.Methods Seventy-three patients with acute pancreatitis caused by gall-stones admitted to Huangmei County People's Hospital from January 2018 to October 2020 were conveniently selected as the study objects and were divided into two groups according to different operation timing.Early group(35 cases):laparoscopic cholecystectomy was performed within 48 h.Delayed group(38 patients):laparoscopic surgery was per-formed 2-8 weeks after remission and stability of acute pancreatitis.The surgical indexes,biochemical indexes,stress indexes,intraoperative conversion rate,postoperative complications and recurrence rate were compared between the two groups.Results The operation time of the early group was longer than that of the delayed group,but the total hos-pital stay was shorter than that of the delayed group,and the differences were statistically significant(both P<0.05).3 d after operation,the levels of various biochemical indexes in both groups were lower than those before operation,and the differences were statistically significant(all P<0.05).However,there was no significant difference in levels of bio-chemical indexes between the two groups(all P>0.05).1 d after surgery,the levels of interleukin-6 and C-reactive protein in early group were lower than those in delay group,and the levels of interleukin-10 were higher than those in delay group,and the differences were statistically significant(all P<0.05).The total incidence of postoperative compli-cations in the early group was 5.71%,lower than that in the delayed group(23.68%),and the difference was statisti-cally significant(χ2=4.597,P<0.05).There was no significant difference in the conversion rate of laparotomy between the two groups(P>0.05).During 1-year follow-up,there was no loss of patients,and there was no acute pancreatitis in the early group,the recurrence rate in the early group was lower than that in the delayed group,and the difference was statistically significant(P<0.05).Conclusion Within 48h,laparoscopic surgery was given to patients with acute pan-creatitis caused by gallstones as early as possible has definite clinical results,can rapid postoperative recovery,low complication rate,is a practical operation.
