目的 研究基于舒适化浅镇静策略(Early Comfort using Analgesia and Minimal Sedation in the ICU,eCASH)理念的护理干预在重症监护病房(Intensive Care Unit,ICU)机械通气患者中的应用价值.方法 随机选取2022年12月—2023年12月贵州省六盘水市钟山区人民医院收治的80例机械通气患者为研究对象,采用密闭信封法分为对照组(n=40)、研究组(n=40).对照组实施常规护理,研究组实施基于eCASH理念的护理干预.对比两组并发症发生情况,护理干预前后的汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)及日常生活能力量表(Activity of Daily Living Scale,ADL)评分.结果 研究组并发症发生率为2.50%,低于对照组(17.50%),差异有统计学意义(χ2=5.000,P<0.05);干预后,研究组HAMA、HAMD评分低于对照组,研究组ADL评分低于对照组,差异有统计学意义(P均<0.05).结论 ICU内机械通气患者实施基于eCASH理念的护理干预,可降低并发症风险,且有助于缓解负性情绪,从而提高患者生活能力.
Effect of Nursing Intervention Based on ECASH Concept on Complica-tions in Patients with Mechanical Ventilation in ICU
Objective To study the application value of nursing intervention based on the concept of early comfort us-ing analgesia and minimal sedation in the ICU(eCASH)in patients with mechanical ventilation in intensive care unit(ICU).Methods A total of patients with mechanical ventilation admitted to Zhongshan District People's Hospital of Liupanshui City,Guizhou Province from December 2022 to December 2023 were randomly selected as study objects and divided into control group(n=40)and study group(n=40)by sealed envelope method.The control group received routine nursing,and the study group received nursing intervention based on eCASH concept.Complications were com-pared between the two groups,and the scores of Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD)and activity of daily living scale(ADL)were compared between the two groups before and after nursing intervention.Results The complication rate of the study group was 2.50%,which was lower than that of the control group(17.50%),and the difference was statistically significant(χ2=5000,P<0.05).After intervention,the HAMA and HAMD scores of the study group were lower than those of the control group,and the ADL scores of the study group was lower than that of the control group,and the differences were statistically significant(all P<0.05).Conclusion Nursing intervention based on eCASH concept in ICU patients with mechanical ventilation can reduce the risk of complications and advan-tageously relieve negative emotions,thus improving patients'life ability.
