目的 评估独活寄生汤应用在肝肾亏虚型腰椎间盘突出症(Lumbar Disc Herniation,LDH)中的临床疗效.方法 随机选取2021年1月—2023年11月北京中医医院怀柔医院收治的70例肝肾亏虚型LDH患者为研究对象,以随机数表法分为对照组(35例,给予腰椎牵引)、观察组(35例,在对照组基础上加用独活寄生汤),比较两组临床有效率、日本骨科协会评估治疗分数(Japanese Orthopaedic Association Scores,JOA)、疼痛视觉模拟评分(Visual Analog Score,VAS)、中医证候评分、炎症介质水平.结果 观察组临床总有效率(97.14%)高于对照组(77.14%),差异有统计学意义(χ2=6.248,P<0.05);治疗前,两组JOA、VAS、中医证候评分、炎症介质水平比较,差异无统计学意义(P均>0.05);治疗2周后,观察组JOA、VAS、中医证候评分、炎症介质水平优于对照组,差异有统计学意义(P均<0.05).结论 对肝肾亏虚型LDH患者实施独活寄生汤治疗,可以降低白细胞介素-1β、肿瘤坏死因子-α、白细胞介素-6水平,缓解疼痛,改善腰椎功能,提高治疗有效率.
Clinical Curative Effect of Duhuojisheng Decoction on Lumbar Disc Her-niation of Liver and Kidney Deficiency Type
Objective To evaluate the clinical effect of Duhuojisheng Decoction in the treatment of lumbar disc hernia-tion(LDH)with liver and kidney deficiency.Methods 70 patients with liver and kidney deficiency LDH admitted to Huairou Hospital of Beijing Hospital of Traditional Chinese Medicine from January 2021 to November 2023 were ran-domly selected as the study objects,and were divided into control group(35 cases,given lumbar traction)and observa-tion group(35 cases,plus Duhuojisheng Decoction)by random number table method.Compared two groups of clinical efficient and the Japanese Orthopaedic Association scores(JOA),Visual Analog Score(VAS),traditional Chinese medicine(TCM)syndrome score,level of inflammatory mediators.Results The total clinical effective rate of observa-tion group(97.14%)was higher than that of control group(77.14%),and the difference was statistically significant(χ2=6.248,P<0.05).Before treatment,there were no statistically significant differences in J0A,VAS,TCM syndrome score,inflammatory mediator level between the wo groups(all P>0.05).After 2 weeks of treatment,JOA,VAS,TCM syn-drome score and inflammatory mediators level in observation group were better than those in control group,and the dif-ferences were statistically significant(all P<0.05).Conclusion Duhuojisheng Decoction can reduce the levels of inter-leukin-1β,tumour necrosis factor-α and interleukin-6,relieve pain,improve the function of lumbar spine,and in-crease the effective rate of treatment for LDH patients with liver and kidney deficiency.
