目的 分析瑞马唑仑复合瑞芬太尼麻醉用于衰弱老年患者胃癌根治术的效果.方法 方便选取潍坊市中医院肿瘤外科于2022年3月—2023年10月收治的88例行胃癌根治术治疗的衰弱老年患者为研究对象,按照随机抽签法分为观察组(n=44,瑞马唑仑复合瑞芬太尼麻醉)和对照组(n=44,咪达唑仑复合瑞芬太尼麻醉).对比两组的麻醉效果.结果 观察组瑞芬太尼用量(2 170.42±105.43)μg少于对照组,意识消失时间(43.02±11.05)s、意识恢复时间(20.51±4.10)min和气管拔管时间(20.68±4.22)min短于对照组,差异有统计学意义(t=3.318、5.299、3.356、3.232,P均<0.05).观察组的术中心血管不良事件发生率(4.55%)和麻醉后监测治疗室(Post Anesthesia Care Unit,PACU)不良反应总发生率(9.09%)均低于对照组(18.18%、27.27%),差异有统计学意义(P均<0.05).结论 在衰弱老年患者胃癌根治术中,采取瑞马唑仑复合瑞芬太尼麻醉有着确切优势.
Effect of Remazolam Combined with Remifentanil Anesthesia for Radical Gastrectomy in Debilitating Gastric Elderly Patients
Objective To study the effect of remazolam combined with remifentanil anesthesia for radical gastrectomy in elderly patients with debilitating gastric cancer.Methods A total of 88 frail elderly patients undergoing radical gas-trectomy treated in the Department of Tumor Surgery of Weifang Hospital of Traditional Chinese Medicine from March 2022 to October 2023 were conveniently selected as the study objects,and they were divided into observation group(n=44,remazolam combined with remifentanil)and control group(n=44,midazolam combined with remifentanil)accord-ing to random drawing method.The anesthetic effect of the two groups was compared.Results The dosage of remifent-anil in the observation group(2170.42±105.43)μg was less than that in the control group,the time of consciousness disappearance(43.02±11.05)s,the time of consciousness recovery(20.51±4.10)min and the time of tracheal extuba-tion(20.68±4.22)min were shorter than those in the control group,and the differences were statistically significant(t=3.318,5.299,3.356,3.232,all P<0.05).The incidence of intraoperative vascular adverse events in the observation group(4.55%)and the total incidence of adverse events in the Post Anesthesia Care Unit(PACU)(9.09%)were lower than those in the control group(18.18%and 27.27%),and the differences were statistically significant(both P<0.05).Conclusion Remazolam combined with remifentanil has definite advantages in radical gastrectomy in elderly patients with debilitating gastric cancer.
